头穴丛刺联合认知训练治疗血管性认知功能障碍临床研究

Clinical Study Cluster Needling of Scalp Acupoints Combined with Cognitive Training in Treatment of Vascular Cognitive Impairment

目的:探讨头穴丛刺联合认知训练对血管性认知功能障碍患者简易智力状况检查法(mini-mental state examination,MMSE)评分、认知功能及生活质量的影响。方法:将125例血管性认知功能障碍患者分为两组,对照组(55例)采用吡拉西坦口服及认知训练与运动训练、作业疗法等常规治疗,研究组(70例)在对照组治疗基础上,采用头穴丛刺治疗,对比两组MMSE评分、认知功能、皮质醇(cortisol,CORT)水平与急性生理与慢性健康评分系统(acute physiology and chronic health evaluationⅡ,APACHE-Ⅱ)评分情况。结果:两组治疗后MMSE评分均较治疗前取得改善,研究组改善优于对照组(P<0.01),且研究组认知功能改善情况显著优于对照组(P<0.05);CORT水平(104.23±35.14)μg·L^(-1)显著低于对照组(125.62±23.14)μg·L^(-1)(P<0.05);另外,研究组APACHEⅡ指标评分改善优于对照组(P<0.01)。结论:头穴丛刺联合认知训练治疗血管性认知功能障碍患者,对其MMSE评分、认知功能及生活质量均具有积极影响。

Objective： To explore the Effects of cluster needling of scalp acupoints Combined with Cognitive Training on MMSE Score,Cognitive Function and Quality of Life in Patients with Vascular Cognitive Impairment. Methods： 125 patients with vascular cognitive dysfunction were divided into two groups. The control group（ 55 cases） was treated with Piracetam Tablets and cognitive and physical training and occupational therapy. The study group（ 70 cases） was treated with cluster needling of scalp acupoints in addition to that of the control group. The MMSE score,cognitive function,cortisol（ CORT） level and acute physiology and chronic health evaluation II（ APACHE-Ⅱ） score of the both groups were compared after treatment. Results： Compared with those of the control group,the improvement of the MMSE score and the cognitive function of the study group were significantly better（ P〈0. 01,P〈0. 05）. The CORT level of the study group（ 104. 23 ± 35. 14） ng·m L^-1was significantly lower than that（ 125. 62 ±23. 14） ng·m L^-1） of the control group（ P〈0. 05）. In addition,the improvement of APACHE II score of the study group was better than that of the control group（ P〈0. 01）. Conclusion： The combination of cluster needling of scalp acupoints combined with cognitive training in treatment of vascular cognitive dysfunction have a significantly positive impact on patients＇ MMSE score,cognitive function and quality of life.
