四逆散合四君子汤化裁方改善肝癌氩氦冷冻术后肝功能损害临床疗效观察

Efficacy Observation of Sini San Combined with Sijunzi Decoction on Treating the Liver Function Lesion after Argon-helium Cryotherapy for Liver Cancer

目的:观察和分析四逆散合四君子汤化裁方治疗肝癌氩氦冷冻术后肝功能损害的疗效。方法:将符合纳入标准的60例肝癌氩氦冷冻术后患者分为中药组与对照组各30例。中药组在对照组治疗基础上加用四逆散和四君子汤化裁方治疗,观察比较两组患者术后肝功恢复情况,记录术后有无肝衰竭等情况,并进行临床数据分析。结果:两组患者均在术后第1天时谷氨酸氨基转移酶(ALT)及天门冬氨酸氨基转移酶(AST)升高,术后第3天及第7天逐渐下降,且不同时间点组内比较,差异有统计学意义(P<0.05);组间比较可知,两组在术前、术后第1天时ALT、AST比较差异均无统计学意义(P>0.05),治疗组术后第7天ALT、AST均显著低于对照组,差异有统计学意义(P<0.05)。总胆红素(TB)在术后第1天数值有升高趋势,但与术前数值比较无统计学意义(P>0.05);术后第3天TB数值升高,术后第7天及术后第21天总胆红素数值下降。两组患者均无出现明显的肝衰竭。结论:四逆散合四君子汤化裁方短期内可改善肝癌氩氦冷冻术后患者肝功能损害的状况,长期服用可保护肝癌患者肝功能,具有一定的临床价值。

Objective ： To observe and analyze the efficacy of Sini San combined with Sijunzi Decoction, which used to treat the liver function lesion after argon-helium cryotherapy for liver cancer. Methods ： Divided the 60 liver cancer patients after argon-helium cryotherapy, who accorded with the inclusive criteria, to traditional Chinese medicine group and control group averagely. In the base of the treatment of the control group, we applied the Sini San combined with Sijunzi Decoction in the traditional Chinese medicine group. And then we observed and compared the postoperative liver function of the two groups, recorded the hepatic failure cases, and analyzed the clinical data. Results： On the 1st postoperative day, both of the alanine aminotransferase （ ALT ） and aspartate aminotransferase （ AST ） of two groups rised. On the 3rd and 7th postoperative day, the two value declined gradually. Compared at different time in the same group, the difference was statistically significant （ P〈0.05 ）. Compared in the interbloek, the difference of ALT and AST has no statistical significance before operation and the 1st postoperative day （ P〉0.05 ）. On the 7th postoperative day, the ALT and AST of treatment group were significantly lower than those in control group. The difference was statistically significant （ P〈0.05 ）. The value of total hiliruhin （ TB ） appeared a rising trend, but it had no statistical significance （P〉0.05）when compared with the preoperative value. The value of TB rises on the 3rd postoperative day, while declining on the 7th day and 21th postoperative day. Both the two groups had no hepatic failure. Conclusion ： Sini San combined with Sijunzi Decoction can improve the liver function lesion after argon-helium cryotherapy for liver cancer in a short time. What＇s more, the treatment can protect the liver fimction after the long-term application.
