基于社区直接挂号三级医院的分级诊疗系统建设探究

长期以来,诊断明确、病情稳定的慢性病、康复期患者难以从三级医院及时转至基层医疗卫生机构,致使急危重症和疑难杂症患者享受不到三级医院的优质医疗资源,而基层医疗卫生机构又门可罗雀,医疗资源综合利用效率和效能低下。为解决上述问题,以镇江市第一人民医院建立由社区直接挂号三级医院的双向转诊系统为例,介绍通过社区医生直接在社区操作挂号三级医院,实现社区和三级医院之间病人信息的共享,方便病人就医,简化由社区转诊到三级医院的流程,实现了上下级医疗机构间的无缝对接,提高了医院医疗服务水平。

For a long time, the patients of clear diagnosis or stable condition chronic disease or in rehabilitation period are unable to go to primary health care institutions timely from the three-grade hospital. In order to solve the above problems, the Zhenjiang First People＇s Hospital achieve a two-way referral system by the community directly registered in the three level hospital, the doctor in the community through the community directly registered in our hospital operation, realize the sharing of patient information between community and the three level hospital, convenient patient, simplified the patients from community referral to t the three level hospital process, to achieve a seamless between the upper and lower levels of medical institutions, improve the medical service level of the hospital.
