Clinical Value of Four-dimensional Ultrasound Imaging in the Evaluation of Fallopian Tube Patency of Patients with Infertility

摘要 目的:以腹腔镜为金标准,评估四维超声造影在检查不孕症输卵管通畅程度中的临床价值。方法:选取滦南县妇幼保健院2012年9月至2015年9月妇科门诊收治的不孕症患者共150名,对其分别进行超声造影检查与宫腔镜检查,以宫腔镜检查为金标准,评定四维超声造影价值。结果:使用宫腔镜对每位患者的双侧输卵管进行手术检查,检测出通畅的敏感度最低,检测出阻塞的敏感度最高。A型诊断准确性最高,B型最低;在进行四维超声造影的过程中,统计了输卵管伞部造影剂喷射情况可以看出,喷射明显时有39例(32.77%)输卵管周围粘连,80例(67.23%)无粘连;无喷射时94例(89.52%)有粘连而11例(10.48%)无粘连。以上两组数据相比之下具有统计学意义,P<0.05。结论:四维超声造影在评估不孕症通畅程度方面意义重大,值得进一步研究和分析。 Objective: With laparoscope as gold standard, to evaluate the clinical value of four-dimensional ultrasound imaging in the examination of fallopian tube patency of patients with infertility. Methods: A total of 150 patients with infertility accepted and treated by outpatient service of Deparmlent of Gynaecology of Maternal and Child Care Service Center of Luannan County from September 2012 to September 2015 are selected. They receive contrast-enhanced ultrasound examination and hysteroscopy examination. With the gold standard of hysteroscopy examination, value of four-dimensional ultrasound imaging is evaluated. Results: Hysteroscopy is used for surgical examination of bilateral fallopian tubes of each patient. The sensitivity of patency is the lowest, and the sensitivity of obstruction is the highest. Type A Diagnosis has the highest accuracy, and Type B Diagnosis has the lowest accuracy. In the process of four-dimensional ultrasound imaging, statistics on spraying of contrast agent by umbrella part of fallopian tube are made. It can be seen that when spraying is obvious, 39 cases (32.77%) have tubal adhesions, while 80 cases (67.23%) have no adhesion; and when there is no spraying, 94 cases (89.52%) have adhesion, while 11 cases (10.48%) have no adhesion. The comparison of above two groups of data is statistically significant. P〈0.05. Conclusion: In the evaluation of patency of infertility, four-dimensional ultrasound imaging is of great significance, and worth further research and analysis.
