后颅凹实质型血管母细胞瘤MRI特征探讨

The Study of MRI Characteristics in the Posterior Fossa Solid Hemangioblastoma

目的研究后颅凹实质型血管母细胞瘤的MRI特征,提高其影像学诊断水平。方法观察经病理证实的60例后颅凹实质型富血供肿瘤(实质型血管母细胞瘤20例,非血管母细胞瘤40例)的血管流空影、DWI信号特点,测量ADC值并进行统计学比较。结果 20例实质型血管母细胞瘤均有血管流空影,DWI像呈等或稍低信号18例,高信号2例,平均ADC值(1.95±0.17)×10^(-3)mm^2/s;40例非血管母细胞瘤有血管流空影6例,无血管流空影34例,DWI像呈等或稍低信号5例,高信号35例,平均ADC值为(0.93±0.13)×10^(-3)mm^2/s。经统计学比较,两者在是否有血管流空影、DWI信号的χ~2值分别为39.231、33.878,P值均<0.05,ADC值间比较t=25.788,P<0.05,差异均有统计学意义。结论存在血管流空影,DWI呈等或稍低信号及对应的较高的ADC值对提示后颅凹实质型血管母细胞瘤有价值。

Objective Study of the features of MRI in the posterior fosssa solid hemangioblastoma, to improve the judg-ment of their characteristics in MRI. Methods We restrospectively analyzed 60 cases of solid tumors with abundant blood supply in posterior cranial fossa （20 cases were solid hemangioblastoma , 40 cases were non-solid hemangioblas- toma） ,which were confirmed pathologically to study their vascular flow voids, signal fealure of DWI and ADC value. Resuits Of 20 cases of the posterior fosssa solid hemangioblastoma there were 20 cases with vascular flow voids, 18 cases of isointense and slightly low signal and 2 cases high signal on DWI, The average of ADC value were （1.95±0.17）×10^-3 mm^2/s）. Of the 40 cases of non-solid hemangioblastoma there were 6 cases with vascular flow voids and 34 cases without. There were 5 cases of isointense and slightly low signal and 35 cases of high signal on DWI, The average of ADC value were （0.93±0. 13 ）×10^-3 name/s, Compared with their vascular flow voids （ X^2 = 39. 231 ）, signal feature of DWI （ X^2 = 33. 878） and ADC value（ t = 25. 788） the distinction has statistically significant（P 〈 0.05）. Conclusion vascular flow voids, isointense and slightly low signal on DWI and higher ADC value be used for the diagnosis of the posterior fosssa solid hemangioblastoma.
