经鼓膜置管鼓室内注入地塞米松联合静脉注射地塞米松治疗重度特发性突聋的效果探讨

目的:探讨用经鼓膜置管鼓室内注入地塞米松联合静脉注射地塞米松法治疗重度特发性突发性耳聋(以下简称重度特发性突聋)的临床效果。方法:将2013年9月至2016年9月期间四川省绵阳市中心医院收治的60例重度特发性突聋患者的临床资料进行回顾性研究。随机将这60例患者分为甲组和乙组,每组各有30例患者。对两组患者均进行常规治疗,在此基础上,对甲组患者采用静脉注射地塞米松法进行治疗,对乙组患者采用经鼓膜置管鼓室内注入地塞米松联合静脉注射地塞米松法进行治疗,然后比较两组患者治疗的总有效率、纯音听阈及其并发症的发生率。结果:1)乙组患者治疗的总有效率高于甲组患者,差异有统计学意义(P<0.05)。2)治疗后,乙组患者的纯音听阈低于甲组患者,差异有统计学意义(P<0.05)。3)治疗后,乙组患者并发症的发生率低于甲组患者,差异有统计学意义(P<0.05)。结论:临床上在对重度特发性突聋患者进行常规治疗的同时,采用经鼓膜置管鼓室内注入地塞米松联合静脉注射地塞米松法对其进行治疗的效果显著,能有效地改善其听力,降低其并发症的发生率。

Objective To study effect of Dexamethasone injection in tympanic cavity through tympanotomy tuhe insertion combine with Dexamethasone intravenous injection for severe idiopathetic sudden deafness. Methods To retrospective study the clinical material of 60 cases of severe idiopathetic sudden deafness treated in Central Hospital of Mianyang City from September 2013 to September 2016,randomly divide 60 cases into Group A and Group B,30 cases each group. Base on conventional therapy, treat Group A with Dexamethasone intravenous injection, treat Group B with therapy of Dexamethasone injection in tympanic cavity through tympanotomy tube insertion combine with Dexamethasone intravenous injection. Compare curative rates, pure tone threshold and rates of complications. Results 1 Curative effect of Group B is higher than Group A, discrepancy of research results were statistically significant （P 〈 0.05）. 2 After treatment, pure tone threshold of Group B is lower than Group A, discrepancy of research results were statistically significant （P 〈 0.05）. 3 After treatment, rates of complications of Group B is lower than Group A, discrepancy of research results were statistically significant （P 〈 0.05）. Conclusion As treat severe idiopathetic sudden deafness with conventional therapy in clinic is effective ,and this therapy can improve patients hearing, reduce rates of complication.
