心脏彩超辅助检查对急性肺栓塞的诊断价值

Diagnostic value of cardiac color ultrasonography as auxiliary examination to patients with acute pulmonary embolism

目的探讨心脏彩超辅助检查对急性肺栓塞患者的诊断价值。方法对72例急性肺俭塞患者临床特征及心脏彩超、血气分析、肺动脉造影、心电图、X射线片、肺CT检查的临床资料进行回顾性分析。结果本组患者存在呼吸困难72例，血氧饱和度降低72例，突发胸闷19例，心动过速、血压下降、头晕15例。应用心脏彩超、血气分析以及肺动脉造影分析辅助检查的急性肺栓塞阳性率均为100％，心电图、X射线片、肺CT检查阳性率分别为38．9％、19．4％、27．8％，心脏彩超、血气分析及肺动脉造影辅助检查的急性肺栓塞阳性率显著高于心电图、X射线片、肺CT检查（Х^2=63．36、97．12、91．32，P〈0.01）。结论对急性肺栓塞患者行心脏彩超辅助检查诊断效果显著且价格低廉，能早期及时明确诊断，对提高患者的治疗成功率，减少化亡牢具有重要意义。

Objective To explore the diagnostic value of cardiac color ultrasonog raphy （CCU） as auxiliary examination to patients with acute pulmonary embolism （APE）. Methods Retrospective analyses were performed on clinical features of 72 APE patients and the clinical data of CCU, blood gas analysis, pulmonary arteriography, ECG, X ray and lung CT. Results Seventy-two ones of the patients developed dyspnea, 72 low blood oxygen saturation, 19 sudden chest distress, 15 tachycardia, falling of blood pressure and dizziness. APE positive rates of CCU, blood gas analysis and pulmonary arte riography as auxiliary examination were 100% respectively, those of ECG, X ray and lung CT 38. 9%, 19.4% and 27.8%, the former significantly higher than the latter （Х^2 =63.36, 97.12, 91.32, P〈0.01）. Conclusion CCU has clinical advantages of higher diagnostic rate and cheapness for APE, could early and promptly diagnose APE, and has crucial significance for boosting the achievement ratio of treatment and reducing death rate.
