硬膜外麻醉下老年无气腹腹腔镜阑尾切除术的安全性及临床治疗效果分析

Analysis on Safety and Clinical Treatment Effect of Gasless Laparoscopic Appendectomy for Elderly Patients under Epidural Anesthesia

目的研究硬膜外麻醉下老年无气腹腹腔镜阑尾切除术的安全性及临床治疗效果。方法选取我院2014年6月至2016年11月收治的80例行阑尾切除术治疗的老年阑尾炎患者,随机分为研究组和对照组,各40例。研究组采用硬膜外麻醉下无气腹腹腔镜阑尾切除术治疗,对照组采用全麻下气腹腹腔镜阑尾切除术治疗。分析比较两组患者的手术相关指标和不良反应。结果两组患者的手术时间和术中出血量比较差异均无统计学意义(P>0.05)。研究组的肛门排气时间、下床活动时间和住院时间均明显短于对照组(P<0.05)。研究组的不良反应发生率明显低于对照组(P<0.05)。结论采用硬膜外麻醉下无气腹腹腔镜切除术治疗老年阑尾炎患者,临床效果显著,能够促进患者早日恢复,且安全性高,值得临床推广应用。

Objective To study the safety and clinical treatment effect of gasless laparoscopic appendectomy for elderly patients under epidural anesthesia. Methods 80 cases of elderly patients with appendicitis underdoing appendectomy in our hospital from June 2014 to November 2016 were selected and randomly divided into study group and control group, with 40 cases in each group. The study group received gasless laparoscopic appendectomy under epidural anesthesia, while the control group received laparoscopic appendectomy under general anesthesia. The surgical relative indicators and adverse reactions of patients were analyzed and compared between two groups. Results No statistical difference was found in the operation time and intraoperative blood loss between two groups （P 〉0.05）. The anal exhaust time, ambulation time and hospitalization time of study group were significantly shorter than those of control group （P〈0.05）. The incidence of adverse reactions of study group was significantly lower than that of control group （P〈0.05）. Conclusions Gasless laparoscopic appendectomy in the treatment of elderly patients with appendicitis under epidural anesthesia has significant clinical effect, can promote the early recovery of patients, with high safety, which is worthy of clinical promotion and application.
