化疗对消化系统恶性肿瘤患者血糖影响的临床观察

Clinical observation of influence by chemotherapy on blood glucose in digestive malignant tumor patients

目的研究化疗对消化系统恶性肿瘤患者血糖的影响。方法 60例消化系统恶性肿瘤患者进行临床研究,患者均进行化疗,分析其化疗前后的血糖相关指标变化。结果初次化疗前,空腹血糖、空腹C-肽、空腹胰岛素分别为(4.61±0.82)mmol/L、(0.95±0.31)μg/L、(7.21±1.21)mIU/L,初次化疗后上述指标分别为(6.42±1.12)mmol/L、(1.85±0.61)μg/L、(11.21±2.99)m IU/L,初次化疗后,患者空腹血糖、空腹C-肽和空腹胰岛素水平均较化疗前显著提高(P<0.05)。4个化疗周期前,空腹血糖、空腹C-肽、空腹胰岛素分别为(5.21±0.53)mmol/L、(1.92±0.48)μg/L、(8.17±2.92)mIU/L,4个化疗周期后上述指标分别为(7.42±1.93)mmol/L、(2.81±1.17)μg/L、(12.31±3.42)mIU/L,4个周期化疗后,患者空腹血糖、空腹C-肽和空腹胰岛素水平均较4个化疗周期前显著提高(P<0.05)。结论消化系统恶性肿瘤的化疗时间与药物均会对糖代谢和胰岛素功能造成影响,监测血糖代谢水平与胰岛功能具有一定的临床意义。

Objective To research infleunce by chemotherapy on blood glucose in digestive malignant tumor patients. Methods A total of 60 patients with digestive malignant tumor were taken into clinical research, and they all received chemotherapy. Analysis was made on changes in blood glucose related indexes before and after chemotherapy. Results Before the first chemotherapy, the patients had fasting blood glucose, fasting C-peptide, and fasting insulin respectively as （4.61 ± 0.82） mmol/L, （0.95 ± 0.31） μg/L and （7.21 ± 1.21） mIU/L, which were respectively （6.42± 1.12） mmol/L, （1.85 ± 0.61） μg/L and （11.21 ± 2.99） mIU/L after the first chemotherapy. The patients had obviously higher fasting blood glucose, fasting C-peptide, and fasting insulin after the first chemotherapy than those before （P〈0.05）. Before completion of 4 cycles of chemotherapy, the patients had fasting blood glucose, fasting C-peptide, and fasting insulin respectively as （5.21 ± 0.53） mmol/L, （1.92 ± 0.48） μg/L and （8.17 ± 2.92） mIU/L, which were respectively （7.42 ± 1.93） mmol/L, （2.81 ± 1.17） μg/L and （12.31 ± 3.42） mIU/L after 4 cycles of chemotherapy. The patients had obviously higher fasting blood glucose, fasting C-peptide, and fasting insulin after 4 cycles of chemotherapy than those before （P〈0.05）. Conclusion Duration and medicine of chemotherapy in digestive malignant tumor both show influence on glycometabolism and insulin function, and monitoring blood glucose metabolism and insulin function contains certain clinical significance.
