静脉留置针自制负压系统在乳腺癌术后皮下积液中的应用

Application of self-made negative pressure system with intravenous indwelling needle in postoperative seroma after breast cancer surgery

目的总结静脉留置针自制负压系统在乳腺癌术后皮下积液治疗中的应用体会。方法乳腺癌改良根治术后出现皮下积液患者113,根据处理方法不同分成传统抽液组、切开置管组和自制负压引流组,观察3组患者不适症状发生率、积液愈合时间、换药频次及操作时间。结果自制负压引流组无不适症状发生率、积液愈合时间、换药频次及操作时间分别为80.0%、(9.16±3.10)天、(2.73±0.69)次和(10.36±2.45)分钟,传统抽液组分别为33.3%、(19.0±13.0)天、(6.40±1.66)次和(17.56±2.22)分钟,两组比较差异有统计学意义(P<0.05)。切开置管组无不适症状发生率、积液愈合时间、换药频次及操作时间分别为62.5%、(9.31±3.26)天、(3.34±0.90)次和(10.38±2.47)分钟,自制负压引流组与切开置管组比较,差异无统计学意义(P>0.05)。结论对乳腺癌术后出现的皮下积液,静脉留置针自制负压系统引流简单易行,治疗效果确切,痛苦小、易于患者接受。

Objective To summarize the application of the self-made negative pressure system with intravenous indwelling needle in postoperative seroma after breast cancer surgery.Methods 113 cases showed postoperative seroma, after modified radical mastectomy of breast cancer.According to different treatment methods, they were divided into 3 groups： the traditional drainage group, the incision and drainage group and the self-made negative pressure drainage group.Observe the incidence of no-symptoms, the seroma healing time,frequency of dressing change and operating time in the 3 groups of patients.Results The incidence of no-symptoms,seroma healing time,frequency of dressing change and operating time were 80%, （9.16 ±3.10） days, （2.73±0.69） and （10.36±2.45） minutes in the self-made negative pressure drainage group, and 33.3% and （19±13） days,（6.40±1.66） and （17.56±2.22） minutes in the traditional drainage group.There was a significant difference between the two groups （P〈0.05）.In the incision and drainage group, the incidence of no-symptoms, seroma healing time, frequency of dressing change and operating time were 80%, （9.16±3.10） days, （2.73±0.69） and （10.36±2.45） minutes.Between the self-made negative pressure drainage group and the incision and drainage group,there was no statistically significant difference （P〉0.05）.Conclusion The drainage by using self-made negative pressure system to cure postoperative seroma after breast surgery is simple and easy to be carried out.It is effective with less pain and easily accepted by patients.
