摘要
目的:分析2009—2014年重庆市卫生资源配置的公平性,为制定优化卫生资源配置政策提供科学依据。方法:运用泰尔指数对重庆市38个区(县)卫生资源配置的公平性进行分析。结果:2009—2014年重庆市卫生资源配置总体公平性较差,卫生机构床位配置的公平性优于卫生技术人员、执业(助理)医师、注册护士配置的公平性。4项评价指标的组内泰尔指数贡献率在43.8%~91.7%的范围内,而组间泰尔指数贡献率在8.3%~56.2%的范围内。结论:重庆市硬件资源配置的公平性优于软件资源。不同区域内部差异是造成重庆市卫生资源配置不公平的主要原因,不同区域之间的差异也对重庆市卫生资源配置的公平性造成影响,且长期以来未得到改善。
Objective: To analyze the equity of health resource allocation in Chongqing from 2009 to 2014 and provide scientific basis for optimizing health resource allocation policies. Methods: The equity of health resource allocation of the 38 districts/counties of Chongqing was analyzed with Theil index. Results: From 2009 to 2014, the overall equity of health resource allocation of Chongqing was relatively poor. Equity of the health institutions beds allocation was better than that of the health workers, practicing ( assistant ) physicians and registered nurses. The contribution rate of inter-group Theil index of four evaluation indexes ranged from 8.3% to 56.2% and the contribution rate of Theil index within the group of four evaluation indexes differed from 43.8% to 91.7%. Conclusion: The equity of the hardware resource allocation is better than that of the software resource allocation in Chongqing. Differences within regions are the main causes of the inequity of the health resource allocation in Chongqing, while differences between regions also affect the equity of the resource allocation and the problem has been unresolved for a long time.
