鼻肠减压管在肠梗阻治疗中的应用价值

The value of nasal bowel decompression tube in the treatment of intestinal obstruction

目的:探讨鼻肠减压管在肠梗阻治疗中的应用价值。方法:选取在2012年5月至2015年5月在广西科技大学第一附属医院接受治疗的103例肠梗阻患者,将其随机分为研究组(52例)和对照组(51例),对照组采用常规的鼻胃减压管进行减压,根据腹部情况继续保守治疗或改手术治疗,研究组则在胃镜下为患者放置鼻肠减压管进行减压,同样根据腹部情况继续保守治疗或改手术治疗。比较两组患者1~3d的引流量、腹围的变化情况、3d后腹痛、腹胀发生率以及减压管的留置时间对治愈有效率的影响。结果:研究组患者1~3d的腹围明显小于对照组患者的腹围,引流量明显的多于对照组患者,差异均有统计学意义(均P<0.05);两组患者放置减压管3d后,研究组患者出现腹痛、腹胀状况的概率明显低于对照组(P<0.05);延迟拔管可以增加患者的治疗有效率,且无论提前还是延迟拔管,研究组相应的治疗有效率均明显高于对照组,差异均具有统计学意义(均P<0.05)。结论:鼻肠减压管在肠梗阻治疗中,能够有效的降低肠内压,改善患者腹痛腹胀的状况。同时,适当的延长拔管时间,能够在一定程度上增加肠梗阻患者的治疗有效率。

Objective： To explore the nasal bowel decompression tube application value in the treatment of intestinal obstruction. Methods： 103 cases of patients with intestinal obstruction in our hospital from May 2012 to May 2015 were selected and randomly divided into an experimental group （52 cases） and a control group （51 cases）. The patients in the control group were treated with routine nasogastric tube decompression, while those in the experimental group received nasogastric tube decompression assisted by gastroscope. 1-3 d drainage volume, abdominal circumference changes, the incidence of abdominal pain and distension after 3 days and decompression tube indwelling time were compared. Results： 1-3 d abdominal girth was significantly lower, and the drainage volume was higher in the experimental group than those in the control group （ P 〈0.05）. 3 d after decompression, the incidence of abdominal pain and distension was significantly lower than that in the control group （ P 〈0.05）. Prolong the extubation time could increase the response rate, and whether early or delayed exhumation, the response rate in the experimental group was higher than that in the control group （ P 〈0.05）. Conclusion： Nasal intestinal decompression tube application in the treatment of intestinal obstruction could effectively reduce the intestinal pressure and improve abdominal distension, and prolonged the extubation time could increase the response rate.
