刮吸法腹腔镜胆囊切除术治疗急性胆囊炎278例治疗分析

Analysis of Laparoscopic Cholecystectomy Surgery by the Curettage and Suction in Treatment of 278 Cases of Patients with Acute Cholecystitis

目的分析刮吸法腹腔镜胆囊切除术治疗急性胆囊炎的治疗价值。方法回顾性分析2012年1月—2016年8月期间在该院接受治疗的278例急性胆囊炎患者资料,将患者分为常规组与刮吸组,每组均为139例患者。为所有患者实施腹腔镜胆囊切除术,刮吸组术中应用刮吸器进行治疗,记录患者的手术治疗效果。结果刮吸组139例患者中,手术成功138例,由于术中出血而中转开腹治疗1例,治疗成功率为99.28%。常规组手术成功133例,术中出血中转开腹6例,治疗成功率为94.11%。在两组患者的手术治疗指征对比方面,刮吸组患者的平均手术时间为(125.65±30.32)min,术中平均出血量为(114.53±31.23)m L,术后平均下床时间为(17.03±3.45)d,均优于常规组患者(P<0.05)。结论刮吸法腹腔镜胆囊切除术治疗急性胆囊炎的效果比较突出,能够有效缓解患者的临床症状,同时临床治疗的有效率相对较高,患者术后康复时间较短,对患者生活质量的提升能够产生积极的影响,建议广泛推广。

Objective To analyze th e v alue of lapa ro scop ic cholecystectomy surgery by th e cu re ttag e an d su c tio n in tr e a t -ment with acute cholecysti tis . Methods 278 c a se s of p a t ien ts with a c u te cholecy sti tis tre a ted in our h o sp i ta l from Jan ua ry 2012 to August 2016 were retrospectively analyzed, and divided into two groups with 139 cases in each, al l pat ients were given the laparoscopic cholecystectomy surgery, the Curet tage and suction group were treated with Curet tage and suction de-vice in operat ion, and the operat ive treatment effect of pat ients was recorded. Results In th e 139 c a se s of p a t ien ts in the Curet tage and suction group, the operat ion of 138 cases was succes sful , and 1 case was given the conversion to laparotomy treatment due to the bleeding in operat ion and the treatment succes s rate was 99.28%, and the operat ion of 133 cases was succes sful in the routine group, and 6 cases were given the conversion to laparotomy treatment due to the bleeding in op-erat ion, and the treatment success rate was 94.11%, and the average operat ive time and average bleeding amount in opera-tion and average time of gett ing out of bed in operat ion in the Curet tage and suction group were respectively （125.65±30.32）min, （114.53±31.23）mL and （17.03±3.45）d, which were bet ter than those in the routine group（P〈0.05）. Conclusion T h e effect of laparoscopic cholecystectomy surgery by the curettage and suction in treatment of pat ients with acute cholecysti tis is out-s tanding, which can effectively relieve the cl inical symptoms of patients and relatively improve the cl inical treatment ef fec-tive rate, and the postoperative rehabi l i tat ion time of pat ients is shorter, which has a positive effect on the improvement of quality of life, and it is suggested to be widely promoted.
