甲氨蝶呤及来氟米特联合应用在类风湿性关节炎治疗中的临床效果评估

Evaluation on Clinical Effect of Combined Application of Methotrexate and Leflunomide in Treatment of Chronic Infectious Arthritis

目的探索甲氨蝶呤与来氟米特联合用于治疗类风湿性关节炎的效果。方法在该院于2014年2月—2016年6月间收治的类风湿性关节炎患者中随机抽取出170例为研究对象,根据治疗方法的不同分入到观察组(87例)和对照组(83例)中,对照组患者采用甲氨蝶呤治疗,观察组患者采用甲氨蝶呤+来氟米特治疗,对比分析两组患者的治疗总有效率、CRP水平、临床症状缓解状况等指标。结果观察组患者的总有效率97.70%比对照组的86.75%高(P<0.05);观察组患者治疗12周后的血清CRP水平[(6.54±1.25)mg/L vs(8.75±1.32)mg/L]明显低于对照组(P<0.05);观察组患者治疗12周后的关节肿胀个数[(5.2±0.7)vs(8.5±1.0)]、晨僵时间[(27.4±5.3)min vs(40.5±6.5)min]比对照组少,关节压痛个数[(6.5±1.2)vs(9.2±1.5)]比对照组短(P<0.05)。结论类风湿性关节炎患者采用甲氨蝶呤联合来氟米特治疗效果确切,有助于提高患者的生活质量,值得推广应用。

Objective To ex plore th e effect of combined ap p l ic a t io n of m etho trex ate and leflunomide in tre a tm en t of ch ronic infect ious a r thr i t is. Methods 170 c a se s of p a t ien ts with ch ro n ic infe ct ious ar th r i t is adm it ted and tre a ted in our h ospi tal from February 2014 to Ju n e 2016 were randomly se le c ted as th e re se a rch objects and d iv id ed into two groups ac co rd in g to d iffer-ent tre atment metho d s, th e control group （ 83 c a s e s） were tre a ted with m e tho trex ate, while th e observa tio n group （87 c a se s） were tre a ted with m etho trex ate and le flu n om ide, an d th e total tre a tm en t effective ra te , CRP leve l an d c l in ic a l symptom r e -lieving con di tio n of th e two groups were compared and an a ly z ed . Results The to ta l effective ra te in th e observa tion group was highe r th an th a t in th e control group （97.70% vs 86.75%）（P 〈0.05）, th e serum CRP leve l in 12 week af te r tre a tm en t in the observa tion group was obviously h ig h e r th an th a t in th e control group [（6.54±1.25）mg/L vs （8.75±1.32）mg/ L]（P 〈0.05）, and the jo in t swelling n um b er and jo in t ten d e rn e s s n um b e r in 12 week af te r tre a tm en t in th e observa tion group were fewer than th o se in th e control g ro u p[（5.2±0.7） vs （8.5±1.0）, （6.5±1.2） vs （9.2±1.5） ] , an d th e morning stiffness time was shorter than tha t in th e control group [（27.4±5.3）min vs （40.5±6.5）m in],（P 〈0.05）. Conclusion The effect of m etho trex ate andle flunomide in tre a tm en t of ch ro n ic infect ious ar th r i t is isd e f in i te, which co n tribu te s to im proving th e q ua lity of life of p a t ie n ts, and it is worth promotion an d a p p l ic a t io n .
