摘要
目的观察3-硝基酪氨酸(3-NT)、缺氧诱导因子-1α(HIF-1α)和8-异前列腺素(8-iso-PGF-2α)在阻塞性睡眠呼吸暂停低通气综合征(OSAHS)合并高血压检测中的意义。方法选择2010年1月至2015年12月就诊的OSAHS患者149例,根据是否合并高血压分为单纯OSAHS组(43例)和OSAHS合并高血压组(106例)。选择同时期行健康体检者35例为健康对照组。观察各组血清3-NT、HIF-1α和8-iso-PGF-2α水平,OSAHS合并高血压患者3-NT、HIF-1α和8-iso-PGF-2α水平与呼吸暂停低通气指数(AHI)的关系,治疗后水平的变化及其各指标的相关性分析。结果 OSAHS合并高血压组和单纯OSAHS组3-NT、HIF-1α和8-iso-PGF-2α水平明显高于健康对照组(P<0.01),而OSAHS合并高血压组明显高于单纯OSAHS组(P<0.01)。OSAHS合并高血压患者的3-NT、HIF-1α和8-iso-PGF-2α水平随着AHI升高而升高(P<0.01),有效治疗后机体的3-NT、HIF-1α和8-iso-PGF-2α水平较治疗前明显下降(P<0.01)。OSAHS合并高血压患者的3-NT水平与HIF-1α(r=0.594,P<0.05)和8-iso-PGF-2α(r=0.647,P<0.05)水平呈正相关,HIF-1α与8-iso-PGF-2α水平呈正相关(r=0.781,P<0.05)。结论 3-NT、HIF-1α和8-isoPGF-2α水平参与了OSAHS疾病的发生发展过程,可以作为OSAHS合并高血压的重要预测因子。
Objective To investigate the significance of 3-nitro-tyrosine ( 3-NT), hypoxia-inducible factor-1α (HIF-1α) and 8-isoprostane (8-iso-PGF-2α) in obstructive sleep apnea hypopnea syndrome (OSAHS) patients with hypertension. Methods One hundred and forty-nine patients with OSAHS who were treated in our hospital from January 2010 to December 2015 were divided into simple OSAHS group ( n = 43) and OSAHS with hypertension group ( n = 106 ) according to whether OSAHS combined with hypertension. At the same time,35 healthy subjects were served as control group. The serum levels of 3-NT , HIF-1α and 8-iso-PGF-2α were detected,and the correlation between the levels of 3-NT ,HIF-1α , 8-iso-PGF-2α and apnea-hypopnea index (AHI) in OSAHS patients with hypertension was analyzed,moreover after treatment, the changes of the levels of these factors were observed,besides, the correlation among these factors was analyzed. Results The serum levels of 3-NT, HIF-1α and 8-iso-PGF-2α in simple OSAHS group and OSAHS with hypertension group were significantly higher than those in control group ( P 〈 0. 01 ) , moreover, which in OSAHS with hypertension group were significantly higher than those in simple OSAHS group ( P 〈0.01) . The levels of 3-NT,HIF-1α and 8-iso-PGF-2αin OSAHS with hypertension group were increased with the increase of AHI ( P 〈 0 .01) , after treatment, the levels of 3-NT,HIF-1α and 8-iso-PGF-2α were significantly decreased, as compared with those before treatment ( P 〈0.0) . The levels of 3-NT were positively correlated to those of HIF-1α ( r = 0 .594,P 〈0.05) and 8-iso-PGF-2α( r = 0.647 , P 〈0.05) ,moreover, the levels of HIF-lawere positively correlated to those of 8-iso-PGF-2α ( r = 0 .781, P 〈0 .05) . Conclusion The 3-NT, HIF-1α and 8-iso-PGF-2α are involved in the pathogenesis and development of OSAHS,which can be regarded as important predictors in patients with OSAHS complicated by hypertension.
