家庭医生签约服务模式对2型糖尿病患者健康管理的效果分析

Signing the family doctor service pattern in patients with type 2 diabetes health management analysis of the effect

目的 :探讨家庭医生签约服务模式对社区2型糖尿病患者健康管理的效果分析,为提高社区2型糖尿病健康管理水平提供依据。方法:选取截至2014年9月在本社区自愿接受家庭医生签约服务管理的2型糖尿病患者1 282例,其中男性564例,女性718例,平均年龄(67±8)岁。签约后接受家庭医生管理服务2年(2014年10月至2016年9月)后,比较签约前和签约管理服务2年后糖尿病患者的服药依从性和空腹血糖控制情况。结果 :签约服务管理后,2型糖尿病患者的服药依从性从签约管理前的72.93%(935/1282)提高到86.35%(1107/1282,P<0.01);空腹血糖控制率从53.59%(687/1282)提高到67.78%(869/1282)(P<0.01)。结论 :家庭医生签约管理服务可明显提升糖尿病患者的服药依从性和血糖控制率,从而能更好提升社区糖尿病患者健康管理的效果。

Objective： Signing the family doctor service mode in type 2 diabetes mellitus patients in community health management analysis, the effect of type 2 diabetes to improve community health management level. Methods： Choose a district north station community through September 2014 voluntary signing the family doctor service management community on 1282 patients with type 2 diabetes, 2 years contract management （October 2014 to September 2016）, compared before and after signing with diabetes medication adherence and fasting plasma glucose control. Results： Research object after contract management, medication adherence to raise from 72.93% to 72.93%, （x2=71.06, P 〈0.01）, the difference was statistically significant; Fasting plasma glucose control rates from 53.59% 53.59%, better than before signing, （x2=54.15, P〈0.01） difference was statistically significant. Conclusion： Family doctors to community diabetes patients after the implementation of contract management, medication adherence and glycemic control in patients with than by the traditional personnel management of public health effect is good, suggested that diabetes health management gradually into the family doctor job category, which can better improve the effect of diabetes patients in community health management.
