肝外胆管癌超声造影和增强MRI影像学特征及诊断价值比较

Comparison of the Imaging Features and Diagnostic Value of Contrast-enhanced Ultrasound and Contrast-enhanced MRI in Extrahepatic Cholangiocarcinoma

目的探讨并比较超声造影(contrast-enhanced ultrasound,CEUS)和增强MRI诊断肝外胆管癌(extrahepatic cholangoocarcinoma,EHCC)的影像学特征及应用价值。方法回顾性分析2014年2月-2016年2月医院经手术确诊的78例EHCC患者病例资料,均行CEUS和增强MRI检查,分析其影像学特征及检查准确率。结果两种检查动脉期增强水平比较差异无显著性(P>0.05),CEUS显示门脉期、延迟期低增强的几率分别为91.03%、100.0%高于增强MRI的10.26%、8.97%(P<0.05);CEUS达到峰值时不均匀增强几率和检查准确率与增强MRI比较差异无显著性(P>0.05)。结论 CEUS、增强MRI诊断EHCC均有特征性表现,且CEUS可作为增强MRI诊断的有益补充,进一步提高检查准确率。

Objective To investigate and compare the imaging features and diagnostic value of contrast-enhanced ultrasound （CEUS） and contrast-enhanced MRI in extrahepatic cholangiocarcinoma （EHCC）. Methods The clinical data of 78 patients with EHCC confirmed by operation in our hospital between February 2014 and February 2016 were analyzed retrospectively. All patients tmderwent CEUS and contrast-enhanced MRI examination. The imaging features and the accuracy rates of examinations were analyzed. Results There was no significant difference in the level of enhancement in arterial phase between tile two groups （P〉0.05）. The probabilities of low enhancement in portal vein phase and delay phase displayed by CEUS （91.03%, 100.0%） were higher than those of MRI （10.26%, 8.97%） （P〈0.05）. There were no significant differences between CEUS and MRI in inhomogeneous enhancement at peak value and the accuracy of detection （P〉0.05）. Conclusion The findings of CEUS and contrast-enhanced MR I are characteristic in the diagnosis of EHCC. CEUS can be used as a useful supplement to MILl diagnosis, further improving the diagnostic accuracy.
