摘要
目的探讨游离脂肪酸(FFA)与冠心病﹙CHD﹚的关系及其临床应用价值。方法检测110例CHD患者、71例疾病对照者和50名健康体检者(正常对照组)血清FFA、同型半胱氨酸(Hcy)、甘油三酯(TG)、总胆固醇(TC)、脂蛋白(a)[Lp(a)]水平。采用受试者工作特征(ROC)曲线评价各项指标诊断CHD的性能。结果 CHD组FFA、Hcy、TG、Lp(a)水平明显高于正常对照组(P<0.01)和疾病对照组(P<0.05、P<0.01),疾病对照组FFA、Hcy、TG、Lp(a)水平明显高于正常对照组(P<0.01),而TC水平3组之间差异均无统计学意义(P>0.05)。FFA诊断CHD的最佳临界值为596.20μmol/L,ROC曲线下面积为0.924,敏感性为85.5%,特异性为90.1%,诊断性能明显优于Hcy、TG、Lp(a)、TC。结论 FFA对CHD的诊断有一定的临床价值。
Objective To investigate the correlation of free fatty acid (FFA)with coronary heart disease (CHD). Methodse A total of 110 patients with CHD,71 disease controls and 50 healthy subjects(healthy control group) were enrolled. Serum FFA,homocysteine(Hcy),triglyceride(TG),total cholesterol(TC) and lipoprotein(a)[Lp(a)] levels were determined. Receiver operating characteristic(ROC)curve was used to evaluate the diagnosis performance of each index. Results FFA,Hcy,TG and Lp(a)levels in CHD group were higher than those in healthy control group(P〈0.01) and disease control group(P〈0.05,P〈0.01),those in disease control group were higher than those in healthy control group (P〈0.01). TC had no statistical significance among the 3 groups (P〉0.05). The optimal cut-off value of FFA for the diagnosis of CHD was 596.20 μmol/L,the area under ROC curve was 0.924,the sensitivity was 85.5%,and the specificity was 90.1%.
