免疫抑制联合抗凝综合疗法治疗抗心磷脂抗体所致复发性流产的效果分析

Effect of comprehensive therapy of immunosuppression and anticoagulant on recurrent miscarriage caused by anticardiolipin antibody

目的观察免疫抑制联合抗凝综合疗法治疗抗心磷脂抗体（ACA）所致复发性流产的效果.方法2014年1月至2016年12月我院收治的235例ACA所致复发性流产患者,随机将其分为对照组143例和观察组92例.对照组给予人绒毛膜促性腺激素治疗,观察组在对照组基础上给予免疫抑制联合抗凝综合疗法治疗.比较两组患者妊娠结局、ACA清除率并观察不良反应发生情况.结果观察组活产率与ACA清除率分别为90.22%、72.83%,均显著高于对照组的73.43%、30.77%,差异均有统计学意义（P〈0.05）;观察组不良反应总发生率与对照组比较,无显著差异（P〉0.05）.结论免疫抑制联合抗凝综合疗法治疗ACA所致复发性流产,可明显提高ACA清除率,改善妊娠结局,且不增加不良反应发生率,具有较高的临床应用价值.

Objective To investigate the effect of comprehensive therapy of immunosuppression and anticoagulant on recurrent miscarriage caused by anticardiolipin antibody （ ACA）. Methods Two hundred and thirty-five cases of patients with recurrent miscarriage caused by ACA in our hospital from January 2014 to December 2016 were selected as research objects, and they were divided into control group （143 cases） and observation group （92 cases）. Patients in the control group were treated with human chorionic gonadotropin, while patients in the observation group were treated with comprehensive therapy of immunosuppression and anticoagulant based on the therapy of the control group. The pregnancy outcome,ACA clearance rate and adverse reactions of the two groups were compared. Results The live birth rate and ACA clearance rate of the observation group were 90.22% and 72.83%,which were higher than 73.43% and 30.77% of the control group , the differences were statistically significant （ P〈0.05）; the adverse reaction rates two groups had no statistically significant （ P〉0.05）. Conclusion The application of comprehensive therapy of immunosuppression and anticoagulant in recurrent abortion caused by ACA can obviously improve ACA clearance rate and pregnancy outcome, and will not increase adverse reaction rate, it has higher clinic application value.
