椎间盘镜和椎板开窗髓核摘除术治疗腰椎间盘突出症的临床分析

Clinical effect of fenestration discectomy and micro-endoscopy discectomy inlumbar disc herniation

目的对椎间盘镜和椎板开窗髓核摘除术治疗单阶段单侧腰椎间盘突出症的效果进行对比研究.方法选择我院收治的单阶段单侧腰椎间盘突出症患者100例作为研究对象,将其分为对照组和试验组,每组50例.试验组患者采用椎间盘镜髓核摘除术进行治疗,对照组患者采用椎板开窗髓核摘除术进行治疗.对两组患者的手术时间、手术出血情况和术后腰椎失稳的发生情况进行对比.结果采用椎间盘镜髓核摘除术的试验组患者的术中出血量显著少于对照组,但手术时间显著长于对照组,差异均具有统计学意义（P〈0.05）.对照组患者的腰椎失稳发生率显著高于试验组（P〈0.05）.结论与椎板开窗髓核摘除术相比,采用椎间盘镜髓核摘除术治疗单阶段单侧腰椎间盘突出症,手术时间较长,但术中出血量较少,术后并发症较低,所以在治疗中可以根据患者的具体状况选择具体的手术方式.

Objective To investigate the clinical effect offenestration discectomy and micro-endoscopy discectomy on lumbar disc herniation. Methods One hundred cases of patients with lumbar disc herniation in our hospital were selected as the research objects, and were divided into control group and experimental group, with 50 cases in each group. Patients in the control group were treated with fenestration discectomy, while patients in the experimental group were treated with micro-endoscopy discectomy. The operation time, surgical bleeding and incidence rate of lumbar vertebral instability of the two groups were compared. Results The surgical bleeding of the experimental group were significantly less than that of the control group, but the operation time of the experimental group were significantly longer than that of the control group （P〈0.05）. The incidence rate of lumbarvertebral instability of the control group was significantly higher than that of the experimental group （ P〈0.05）. Conclusion Compared with fenestration discectomy, micro -endoscopy discectomy has longer operation time in single phase lumbar disc herniation, but has less intraoperative blood loss and lower postoperative complications incidence rate, so the patient＇s specific situation specific operation method can be choosen according to patient＇s specific situation.
