声触诊组织量化技术鉴别诊断甲状腺良恶性结节的价值研究

摘要 目的探讨声触诊组织量化技术(VTQ)在甲状腺良、恶性结节鉴别诊断中的应用价值。方法选取经手术病理证实的105例甲状腺结节患者共计131个结节,应用常规超声及VTQ技术在术前进行扫查,运用统计分析软件对结果进行分析,甲状腺良性、恶性结节组间VTQ值的比较采用两样本t检验,良性、恶性结节与结节周围腺体VTQ值的比较采用配对t检验。根据甲状腺良恶性结节的VTQ值,绘制ROC曲线,确定VTQ值预测甲状腺良恶性结节的最佳界点。结果甲状腺良性结节平均VTQ值与同深度处腺体的VTQ值比较,差异有统计学意义(P<0.05);恶性结节平均VTQ值显著大于同深度处腺体的VTQ值,两者比较差异有统计学意义(P<0.05);良性、恶性结节平均VTQ值比较,差异有统计学意义(P<0.05)。以VTQ值≥2.62 m/s为诊断甲状腺恶性结节的最佳界点,其敏感度、特异度分别为94.8%、77.1%。结论 VTQ有助于鉴别甲状腺良恶性结节,具有广泛的临床应用价值。 Objective To investigate the value of Virtual Touch Tissues Quantification (VTQ) in the differential diagnosis of benign and malignant thyroid nodules. Methods 105 patients ( 131 thyroid nodules) were examined by conventional ultrasound and VTQ. The pathological diagnosis served as the golden standard. The operating characteristic curve ( ROC curve) was made according to VTQ values of benign and malignant thyroid nodules. Results There was statistical difference between the nodules and the peripheral thyroid tissues for benign nodules (P 〈 0.05), and malignant nodules (P 〈 0.05 ) respectively. Differences between benign and malignant nodules VTQ values was statistically significant ( P 〈 0.05 ). The best cut - off values of VTQ was 2.62 for diagnosis. The diagnostic sensitivity and diagnostic specificity of malignant thyroid solid nodules was respectively 94.8% and 77.1%. Conclusion VTQ has important clinical significance and helpful in differentiating benign and malignant thyroid nodules.
