TOAST分型的急性缺血性脑卒中患者血糖变异性及其和预后的关系研究

The relationship between blood glucose variability and prognosis in patients with acute ischemic stroke TOAST

目的研究TOAST分型的急性缺血性脑卒中患者血糖变异性及其和预后的关系。方法选取本院2014年2月~2015年2月接诊的112例急性缺血性脑卒中患者作为研究对象,据血糖监测将其分为血糖变异组和非血糖变异组,对所有患者的TOAST分型各亚型血糖变异性、TOAST分型各亚型血糖变异性两两比较情况、NIHSS评分以及Barthel指数、患者预后的多因素进行分析。结果 2组患者的年龄、糖尿病史以及NIHSS评分患者例数具有明显差异(P<0.05)。血糖变异组患者的LAA患者例数明显多于非血糖变异组(P<0.05),LAA组血糖变异性与SAO组和CE组比较有明显差异(P<0.05),而SAO组和CE组的血糖变异性没有明显差异(P>0.05)。入院半个月后血糖变异组患者的NIHSS评分以及Barthel指数没有明显差异(P>0.05),而非血糖变异组患者的NIHSS评分以及Barthel指数均有明显的变化,且2组患者在入院半个月后的NIHSS评分以及Barthel指数有明显差异(P<0.05)。年龄、NIHSS评分以及血糖变异等因素与急性缺血性脑卒中患者预后有关。结论年龄、糖尿病史、LAA患者等均是影响患者血糖变异性的相关因素,且血糖变异性对急性缺血性脑卒中患者的预后具有一定的影响,是其独立的危险因素,即血糖变异性异常的急性缺血性脑卒中患者的恢复较缓慢,预后情况不良。

Objective To study the relationship between blood glucose variability and prognosis in patients with acute ischemic stroke of TOAST type.Methods 112 cases of acute ischemic stroke patients in our hospital from February 2014 to February 2015 were as the research object,according to the blood glucose results of the monitoring,they were divided into the study group and the control group,all patients with gender and age in general and TOAST subtypes of blood glucose variability,variability of blood glucose of two two TOAST subtypes,NIHSS score.Compared the prognostic factors for patients with Barthel index.ResultsThe two groups of patients,age,history of diabetes and the number of patients with NIHSS scores were significantly different（P〈0.05）.The number of patients in the study group,patients with LAA was significantly higher than that of the control group,it was with statistical significance（P〈0.05）,TOAST classification type sub type glucose variability compared,the LAA blood glucose variability was significantly for Sao group and CE group,the difference was statistically significant（P〈0.05）,and Sao group and CE group of glucose variability of no significant difference,no statistical significance（P〉0.05）.After half a month of admission,the patients in study group of the NIHSS score and Barthel index had no significant difference,while the control group patients NIHSS score and Barthel Index were significantly change and two groups of patients in half a month after admission NIHSS score and Barthel index had obvious differences,it was with statistical significance（P〈0.05）.Age,NIHSS and glucose variability factors were influence the prognosis of patients with acute ischemic stroke.Conclusion The age,history of diabetes,LAA and are related factors that influence the variation of blood glucose,and blood glucose variability in patients with acute ischemic stroke prognosis has certain influence,is the independent risk factors,namely the variation of blood glucose abnormalities of acute ischemic stroke patients with recovery is slow,the prognosis is poor.
