Zhang Yu's Experiences in the Diagnosis and Treatment of IgA Nephropathy on the Basis of “Deficiency-Wind-Stasis-Toxin” Complicated Pathogenesis Network

"虚—风—瘀—毒"复杂病机网络能较好地反映IgA肾病病机的复杂性及其病机的总体演变过程。其中"脾肾两虚"为本,是IgA肾病发病内在因素,"外风"为始动因素或诱因,"伏风"是导致IgA肾病迁延难愈,病情反复的基本因素,而"内风"的形成,络脉中的"瘀血",以及"毒邪"的化生是加重肾脏病进展,导致病情恶化的继发性因素。治疗应益肾健脾复其本,兼顾他脏以截断;外风宜散,伏风须托,络深当搜,内风当熄;起始治络,全程治络,化瘀宜辨轻重;注重清热解毒法与泄化浊毒法的运用。

The ＂deficiency-wind-stasis-toxin＂ complicated pathogenesis network reflects the complexity of IgA nephropathy pathogenesis and its overall evolution process.The deficiency of spleen and kidney is the root and the internal factor of IgA nephropathy.The external wind is the initial factor or inducing factor.Hidden wind is the basic factor inducing the lingering situation and the recurrence of the disease.The formation of internal wind,the stasis in meridians and the transformation of toxic pathogen aggravate the development of nephropathy,resulting in the secondary factors of deterioration.In the treatment,benefiting the kidney and strengthening the spleen are the basic principle,combined with the functional regulation of the other organs.The wind pathogens are eliminated accordingly.The dysfunction in meridians and collaterals is regulated in the entire treatment.The stasis is removed in compliance with the differentiation.In the treatment,the therapeutic method for clearing heat and detoxification and that for resolving turbid fluids and detoxification are carefully concerned.
