摘要
目的 回顾性分析化学治疗联合肠内营养治疗对食管癌、贲门癌营养不良患者的疗效及安全性.方法 收集2014年12月至2016年4月解放军沈阳军区总医院收治的首次接受化学治疗的60例晚期食管癌、贲门癌营养不良患者的临床资料,分为联合治疗组(32例)和单纯化学治疗组(28例).单纯化学治疗组采用化学治疗,联合治疗组在此基础上联合肠内营养治疗.观察比较两组患者治疗前后的体质量、白蛋白、前白蛋白、血红蛋白,以及并发症(腹胀、腹泻、恶心呕吐)等发生情况.结果 联合治疗组在营养支持治疗后与单纯化学治疗组比较,体质量、白蛋白、前白蛋白、血红蛋白明显升高,差异有统计学意义(P〈0.01),Ⅱ级以上的消化道不良反应明显较少,差异有统计学意义(P〈0.05).结论 化学治疗联合肠内营养治疗能有效改善食管癌 、贲门癌患者的机体营养状况,提高免疫力,减少因化学治疗引起的胃肠道不良反应,提高患者对抗癌治疗的耐受性,值得临床推广.
Objective To investigate the curative effect,safety and feasibility of enteral nutrition for improving nutritional condition of esophageal and cardiac carcinoma patients undergoing chemotherapy. Methods A retrospective study was performed on 60 malnutrition patients with esophageal and cardiac carcinoma who were admitted from December 2014 to April 2016. Patients were divided into the treatment group(n=32)and the control group(n=28). Patients in the treatment group received chemotherapy,the others in the control group received chemotherapy combined with enteral nutrition. The weight,albumin,pre-albumin,hemoglobin and complications(abdominal distention,diarrhea,nausea and vomiting) were moni-tored before and after chemotherapy,respectively. Results The weight,albumin,prealbumin and hemoglo-bin in the treatment group were significantly higher than those in the control group(P〈0. 01),complica-tions were significantly reduced(P〈0. 01). Conclusion Early postoperative enteral nutrition therapy can improve the nutritional status and immunity. It also can reduce the gastrointestinal adverse reaction caused by chemotherapy and improve patient tolerance of anti-cancer treatment,and it is worthy of clinical promotion.
