改良横一字法内眦赘皮矫正术

Modified Transverse-one's Procedure for the Epicanthoplasty

目的:在横一字法矫正内眦赘皮基础上,探求一种更为符合眼形特点及可有效减少皮肤张力的内眦赘皮矫正术式,使术后切口瘢痕不显,外形美观。方法:首先确定原内眦角在皮肤表面的投影点为新的内眦点,改变以往水平横切赘皮线为斜向上约15°~45°,切开赘皮线后,切口线斜向下经过原内眦点后,沿下睑睫毛缘向外延伸,使赘皮松解后呈一个小三角形皮瓣以回填内眼角下方组织退缩后的小凹陷,同时避免切口上方皮肤皱褶的形成,切口在无张力下呈横一字型缝合。结果:本组300例就医者,采用改良横一字法行内眦赘皮矫正,术中皮肤缝合后基本无皮肤皱褶(小猫耳)生成,随访6个月~1年,内眦角形态自然,泪埠显露适中,瘢痕不显。结论:采用改良横一字法矫治内眦赘皮,设计简单,操作容易,瘢痕不显,术后内眦形态满意。

Objective To explore and modify epicanthoplasty by the transverse-one＇s procedure that can effectively reduce skin tension to make postoperative scar inconspicuous and appearance beautiful. Methods The projection point of the original medial canthus on the skin surface was determined along the lower palpebral edge and marked as the new epicanthus point. The previous epicanthus traverse was changed to be obliquely upward by approx 15°-45° to make the loosen epicanthus be a small triangular flap. The incision line was extended outwards below the lower palpebral edge when the epicanthus was tightened to the nasal side. The adhesive tissue between the orbicularis, epicanthus ligament and fascia below the incision were separated to restore the dislocated skin and the incision was sutured in transverse-one＇s form without tension. Results No cat ear was formed after the skin suture in all 300 patients underwent modified transverse-one＇s procedure. The patients were followed up 6 months to 1 year and satisfactory results obtained. Conclusion Modified transverse-one＇s procedure for the epicanthoplasty is characterized by simple design, easy operation, inconspicuous scar and good postoperative appearance.
