绷带卷加压包扎预防起搏器植入术后囊袋出血的应用效果

The effects of bandage rollers compression on prevention of pocket bleeding after pacemaker implantation

目的探讨绷带卷加压包扎在永久心脏起搏器植入术后囊袋止血中的应用效果。方法选取永久心脏起搏器植入患者591例,分为试验组和对照组。试验组(296例)用两个医用绷带卷并排放置于起搏器囊袋上方,与切口平行,3M胶布条固定绷带卷,松紧以伤口不出血、绷带卷无移位为准,术后6h松解胶布条和绷带卷。对照组(295例)将1 000g沙袋置于起搏器囊袋上方,术后6h取走沙袋。比较两组患者压迫止血的安全性和有效性。结果两组在切口出血、囊袋血肿和皮肤压力性损伤发生方面比较差异无统计学意义(P>0.05);试验组切口疼痛、尿潴留和睡眠障碍的发生率低于对照组,比较差异有统计学意义(P<0.05)。结论绷带卷加压包扎法可明显减少患者伤口疼痛、尿潴留、睡眠障碍的发生。

Objective To evaluate the effect of bandage rollers compression on pocket hemostasis in patients after implantation of permanent pacemaker.Methods A total of 591 patients after implantation of permanent pacemaker were enrolled in this study and divided into trial group and control group.The wounds of 296 patients in trial group received compression by two bandage rollers,which were placed parallelly on the wound side by side,above the pocket of pacemaker,fixed by 3Madhesive tapes so that no bleeding of wound and no shifting of rollers.6hours after the operation,the tapes and bandage rollers were released.The wounds of 295 patients in control group received compression by 1 000 g sand bag which were taken off 6hours after the operation.The safety and effects of hemostasis in patients between the two groups were compared.Results The incidences of incision bleeding,pocket hematoma and pocket skin pressure injury were not significantly different between the two groups（P〉0.05）.The incidences of postoperative pain,urinary retention and insomnia were significantly different between the two groups（P〈0.05）.ConclusionThe incidences of postoperative pain,urinary retention and insomnia are significantly reduced in patients who received bandage roller compression.
