甘露醇在控制高血压性脑出血患者早期血肿扩大中的影响评估

Effect assessment of mannitol on early hematoma enlargement in patients with hypertensive intracerebral hemorrhage

目的探讨在临床中应用甘露醇治疗高血压性脑出血患者对早期血肿扩大的影响。方法针对我院收治的2015年1月~2016年9月50例高血压性脑出血患者,依据临床中采取脱水降颅压治疗方法的不同,分为研究组(25例)、对照组(25例)。对照组患者临床中应用常规药物进行治疗,研究组患者应用甘露醇治疗,分析比较两组患者早期血肿扩大的变化。结果研究结果证实,在两组患者中,对照组患者血肿形态不规则占54.2%,研究组患者血肿形态不规则占10.0%;两组收缩压比较,有统计学差异(P<0.05);研究组患者应用甘露醇治疗后,患者纤维蛋白原水平降低,两组比较有统计学差异(P<0.01);同时,研究组中高血压性脑出血患者的早期血肿未明显扩大,临床治疗过程中血肿未扩大的病例数明显多于对照组,两组治疗效果比较,差异有统计学意义(P<0.05)。结论针对临床中高血压性脑出血患者,在治疗疾病中应用甘露醇药物,可以帮助控制患者早期血肿扩大,临床治疗效果显著,可以有效提升临床治疗疗效,发挥积极应用价值。

Objective To explore the effect of mannitol in the treatment of early hematoma enlargement in patients with hypertensive intracerebral hemorrhage. Methods A total of 50 cases of hypertensive intracerebral hemorrhage in our hospital from January 2015 to September 2016 were divided into study group（25 eases） and control group（25 eases） according to the different treatment methods of dehydration and reduction of intracranial pressure. The patients in the control group were treated with conventional drugs. The patients in the study group were treated with mannitol. The changes of early hematoma enlargement in the two groups were analyzed, Results The results of the study confirmed that the patients with hematoma irregularities in the control group accounted for 54.2%, and the patients with hematologic malformations in study group accounted for 10.0%. The systolic blood pressure between the two groups was statistically significant （P〈0.05）. The level of fibrinogen was reduced in the study group after treatment with mannitol. There were statistically significant differences between the two groups （P〈0.01）. At the same time, the early hematoma of patients with hypertensive intracerebral hemorrhage was not significantly enlarged in the study group. The number of cases of hematoma enlargement during clinical treatment in the study group was significantly higher than that in the control group, and the difference between the two group was signifieant（P〈0.05）. Conclusion It is concluded that the application of mannitol in the treatment of patients with hypertensive intracerebral hemorrhage can help to control the early hematoma enlargement, with remarkable clinical treatment effect, which can effectively improve the clinical therapeutic effect and has a positive application value
