Braden评分对急性脑卒中患者失禁相关性皮炎的预测价值及护理效果分析

Predictive value and nursing effect of Braden score on incontinence-associated dermatitis in patients with acute stroke

目的探讨Braden评分在急性脑卒中患者失禁相关性皮炎中的预测价值及护理效果。方法取2013年12月~2016年11月医院收治急性脑卒中患者70例,入选患者均采用Braden评分对失禁相关性皮炎进行评估、预测,对于高危患者采取有效的措施进行护理干预。结果急性脑卒中患者IAD Braden评分≤12分、13~14分的,高于非IAD患者(P<0.05);急性脑卒中患者IAD患者Braden评分15~16分,低于非IAD患者(P<0.05);急性脑卒中合并IAD患者与非IAD Braden评分中摩擦力、剪切力比较差异无统计学意义(P>0.05);急性脑卒中合并IAD患者Braden评分中感知能力、潮湿度、移动能力、营养摄取、活动能力,低于非IAD患者(P<0.05);多因素Logistic回归分析结果显示:急性脑卒中合并IAD患者与Braden能力评分中潮湿度、移动能力、营养摄取能力、活动能力关系密切(P<0.05)。结论将Braden评分用于急性脑卒中患者中效果理想,能预测失禁相关性皮炎发生率,为临床护理方案制定提供参考。

Objective To explore the predictive value and nursing effect of Braden score on incontinence-associated dermatitis in patients with acute stroke. Methods A total of 70 patients with acute stroke admitted in hospital from December 2013 to November 2016 were enrolled in the study. The incontinence-related dermatitis of the patients were evaluated and predicted by Braden score. The effective measurement for nursing intervention was taken for high-risk patients. Results The number of IAD patients with Braden score ≤ 12 points, 13-14 points was more than that of the non-IAD patients among the patients with acute stroke （P〈0.05）. The number of IAD patients with Braden score 15 to 16 points was less than that of the non-IAD patients among the patients with acute stroke （P〈O.05）. There were no significant differences in the frictional force and shear force of the Branden score between acute stroke patients with IAD and non-IAD（P〉0.05）. The sensitivity, humidity and movement ability,nutrient intake and activity ability of Braden score in acute stroke patients with IAD were lower than those of Braden score in acute stroke patients with non-IAD （P〈0.05）. Multivariate logistic regression analysis showed that patients with acute stroke combined with lAD were closely related to the degree of humidity, mobility, nutrient intake and activity in Braden score（P〈0.05）. Conclusion The Braden score has an ideal effect for patients with acute stroke, which can and provide reference for clinical nursing program. predict the incidence of incontinence-associated dermatitis.
