腹透相关性腹膜炎患者血清万古霉素谷浓度与临床疗效的关系

Relationship between serum vancomycin trough concentration and clinical efficacy in the patients with peritoneal dialysis-related peritonitis

目的分析腹膜透析相关性腹膜炎患者的万古霉素谷浓度及临床疗效。方法对2015年1月~2016年10月于温州市人民医院诊断为腹膜透析相关性腹膜炎并监测万古霉素谷浓度的患者进行筛选,对入选的21例患者按谷浓度水平分为两组。A组谷浓度水平<10 mg/L,B组谷浓度水平≥10 mg/L;收集相关临床资料并进一步分析,同时比较两组患者的临床有效率、住院时长的差异。结果两组临床有效率分别为50.00%、76.92%,差异无统计学意义(P>0.05),但随着谷浓度水平的增高,住院时长[(28.37±4.63)天vs(23.69±4.21)天]缩短,两组住院时长差异有统计学意义(P<0.05)。结论万古霉素不同谷浓度水平时临床有效率不变,但提高万古霉素谷浓度水平能降低住院时长,同时可能减轻患者的经济负担。

Objective To analyze the trough concentration of vaneomyein and its clinical efficacy in the patients with peritoneal dialysis-related peritonitis. Methods The patients who were diagnosed as peritoneal dialysis-related peri- tonitis and were given the monitoring of vaneomyein trough concentration in Wenzhou People＇s Hospital from January 2015 to October 2016 were screened. 21 enrolled patients were divided into two groups according to the trough concentration. In group A, the trough concentration level was 〈10 rag/L; in group B, the trough concentration level was ≥10mg/L; the relevant clinical data were collected and further analyzed. At the same time, the differences of clinical effective rate and the length of stay between the two groups were analyzed. Results There was no statistically significant difference in the clinical effective rate （50.00% vs. 76.92%） between the two groups （P〉0.05）. But with the increase in the trough concentration, the length of stay [（28.37±4.63） days vs. （23.69±4.21） days] was shorter, and the difference was statistically significant （P〈0.05）. Conclusion The clinical effective rate of vaneomycin at different trough concentration levels is the same, but increasing the trough concentration level of vancomyein can reduce the length of stay, while the patient＇s financial burden may be lessened.
