银屑病患者血液学检验指标特点及临床意义

Characteristics and clinical significance of hematological indices in the patients with psoriasis

目的探讨银屑病患者血液学检验指标特点及临床意义。方法选择2015年1月~2016年12月在我院就诊的银屑病患者380例为研究对象。另选择同期健康体检者100例为对照组。比较两组血常规、血生化指标、细胞因子水平、免疫指标等。结果 (1)血常规结果比较:银屑病组WBC、N%、N、EO%、EO、PLT、MPV显著高于对照组,差异有统计学意义(P<0.05);银屑病组LYMPH、MONO%、PDW显著低于对照组,差异有统计学意义(P<0.05)。(2)血生化指标比较:银屑病组TP、ALB、HDL、Apo A1、CA显著低于对照组,TG、LDL、L(a)、Apo B、UA显著高于对照组,差异有统计学意义(P<0.05)。(3)免疫指标比较:银屑病组Ig G、CRP、ESR显著高于对照组,差异有统计学意义(P<0.05)。(4)细胞因子水平比较:银屑病组s IL-2R、IL-2、IL-4、IL-10、IL-12水平均显著高于对照组,差异有统计学意义(P<0.05)。结论银屑病的发病机制复杂,患者存在炎症反应增强、血细胞计数异常、细胞因子水平异常、免疫功能紊乱、低蛋白血症倾向、高脂血症、高尿酸血症倾向等。

Objective To investigate the characteristics and clinical significance of hematological indices in the pa- tients with psoriasis. Methods A total of 380 patients with psoriasis who were diagnosed in our hospital from January 2015 to December 2016 were selected as the study subjects. Another 100 healthy subjects were selected as the control group. The blood routine test, blood biochemical indices, cytokine levels, and immune indicators were compared between the two groups. Results （1）Comparison of blood routine test： The WBC, N%, N, EO%, EO, PLT and MPV in the psoriasis group were significantly higher than those in the control group, and the differences were statistically significant（P〈0.05）; the levels of LYMPH, MONO% and PDW in the psoriasis group were significantly lower than those in the control group, and the differences were statistically significant （P〈0.05）.（2）Comparison of blood biochemical indices： TP, ALB, HDL, ApoA1, CA in the psoriasis group were significantly lower than those in the control group, and TG, LDL, L （a）, ApoB, UA were significantly higher than those in the control group. The differences were statistically significant （P〈0.05）. （3）Comparison of the immune indices：The levels of IgG, CRP and ESR in the psoriasis group were significantly higher than those in the control group, and the differences were statistically significant（P〈0.05）. （4）Comparison of the cytokine level： The levels of SIL-2R, IL-2, IL-4, IL-10 and IL-12 in the psoriasis group were significantly higher than those in the control group, and the differences were statistically significant （P〈0.05）. Conclusion The pathogenesis of psoriasis is complex. The patients have increased inflammatory response, abnormal blood cell count, abnormal levels of cytokines, immune dysfunction, hypoproteinemia tendency, hyperlipidemia and hyperuricemia tendency, etc.
