进展性脑梗死临床病因分析

Clinical etiology of progressive cerebral infarction

目的对进展性脑梗死临床病因进行详细探究。方法随机选取2014年5月至2016年10月本院收治的100例急性脑梗死患者作为研究对象,有30例患者为进展性脑梗死患者,作为本次研究观察组,另外70例患者为非进展性脑梗死患者,作为本次研究对照组。两组患者在性别、学历等一般资料方面比较,差异不具有统计学意义。对100例患者的年龄、降压过低或过快、冠心病史、糖尿病史、颅内动脉狭窄或闭塞、短暂性脑缺血发作、电解质紊乱、高脂血症、纤维蛋白原增高、D-二聚体增高、C反应蛋白增高等因素进行统计和分析。结果在18个因素中,有5个因素差异具有统计学意义,P<0.05,即降压过低或过快、糖尿病史、颅内动脉狭窄或闭塞、短暂性脑缺血发作、纤维蛋白原增高;另外,其他因素差异不具有统计学意义,P>0.05。结论根据本次研究,造成进展性脑梗死的临床病因主要有降压过低或过快、糖尿病史、颅内动脉狭窄或闭塞、短暂性脑缺血发作、纤维蛋白原增高。

Objective To investigate the clinical etiology of progressive cerebral infarction. Methods A total of 100 patients with acute cerebral infarction who were admitted to our hospital from May 2014 to October 2016 were enrolled, among whom 30 had progressive cerebral infarction （observation group） and 70 had non-progressive cerebral infarction （control group）. There were no significant differences in general information including sex and education background between the two groups. All the patients were analyzed in terms of the factors including age, great or rapid reduction in blood pressure, a history of coronary heart disease, a history of diabetes, intracranial artery stenosis or occlusion, transient ischemic attack, electrolyte disturbance, hyperlipidemia, and increases in the levels of fibrinogen, D- dimer, and C-reactive protein. Results There were significant differences between the two groups in five factors, i.e., great or rapid reduction in blood pressure, a history of diabetes, intracranial artery stenosis or occlusion, transient isehemie attack, and increased fibrinogen （P〈0.05）. There were no significant differences in the other factors between the two groups （P〉0.05）. Conclusion The major causes of progressive cerebral infarction are great or rapid reduction in blood pressure, a history of diabetes, intraeranial artery stenosis or occlusion, transient isehemie attack, and increased fibrinogen.
