基于COSO-ERM的老年病医院病人安全风险管理模型研究

Study on safety risk management model of geriatric hospital based on COSO-ERM

[目的]构建基于《企业风险管理——整合框架》(COSO-ERM)的老年病医院病人安全风险管理模型,探索老年病医院全面安全风险管理体系。[方法]应用COSO-ERM三维立体框架的四大目标、八大要素、四大层面构建关于老年病医院病人安全风险管理模型条目池,采用集体讨论法和专家访谈法形成专家咨询问卷,采用德尔菲专家咨询法,对20名专家进行两轮专家咨询,以确定老年病医院安全风险管理模型。[结果]两轮专家咨询问卷回收率分别为90.91%、100.00%,专家权威系数(Cr)为0.81,两轮函询过后指标的变异系数为0.06~0.20,均小于0.25,表示专家的意见较为集中,拟定出安全风险管理模型共3个维度30个条目。[结论]基于COSO-ERM采用德尔菲专家咨询法构建的老年病医院病人安全风险管理模型,研究方法科学可靠,为老年病医院全面安全风险管理提供依据。

Objective： To construct the safe risk managementmodel of the elderly patients in the geriatric hospi-tal based on “the enterprise risk management integrated framework”（ COSO -ERM）and to explore the com-prehensive safety risk management system of the geriatric hospital. Methods： Four objectives, eight elements and four levels of the COSO-ERM three-dimensional framework were used to construct thtient safety risk management model of the geriatric hospital. The expert consultation questionnaire was formed by collective discussion and expert rnterview. Using the Delphr method, through two rounds of 20 experts, ad-vice to determine the model of geriatric hospitals safety risk management. Results： The recovery rate of the two rounds of expert consultation questionnaires were： 90. 91 % and 100. 00 % , respectively. Expert authority coefti- cient（CR）was 0. 81. The smaller coefficient of variation, the better coordination of experts. After two rounds of expert consultation, the coefficient of variation in the indexwas from 0.06 to 0. 20, which was less than 0. 25？ in-dicating that the expert＇s opinion was more concentrated. The security risk management model was composed of 30 items in three dimensions. Conclusions： Based on COSO -ERM,Delphi’ s expert consultation method was used to construct the model of patient＇s safety risk management in the geriatric hospital. The research methods were scientific and reliable,which provided the basis for comprehensive safety risk management of the geriatrichospital.
