IL-1、IL-18、hs-CRP与脑卒中后抑郁的相关性调查研究 被引量:11

摘要 目的:探讨白细胞介素-1(IL^(-1))、白细胞介素-18(IL^(-1)8)、超敏C反应蛋白(hs-CRP)与脑卒中后抑郁的相关性,为下一步的早期预防干预措施提供一定的指导以及依据。方法:将102例脑卒中后抑郁(PSD)的患者纳入到观察组,并且随机选取100例没有并发PSD的脑卒中患者纳入到对照组,对于两组患者血清的IL^(-1)、IL^(-1)8、hs-CRP等相关炎症因子进行测定,并观察其与SDS评分之间的关系。结果:通过对于两组患者血清的IL^(-1)、IL^(-1)8、hs-CRP等炎症因子的进行测定和对比发现,观察组患者血清IL^(-1)、IL^(-1)8、hs-CRP水平较对照组患者明显升高,两组患者的对比差异具有统计学意义(P<0.05),且重度PSD患者中相关炎症因子水平的变化均高于轻中度患者,差异具有统计学意义(P<0.05)。经Logistic回归分析发现,炎性因子IL^(-1)、IL^(-1)8、hs-CRP是脑卒中后抑郁症发生的危险因素,差异具有统计学意义(P<0.05)。结论:根据患者血清IL^(-1)、IL^(-1)8、hs-CRP水平预测患者脑卒中后是否会伴发抑郁,加强临床观察,做到早发现、早治疗,可指导临床用药,从而有效的治疗卒中后抑郁,促进患者的康复。 Objective To explore the correlation between IL^(-1), IL^(-1)8, hs-CRP and post-stroke depression, so as to provide some guidance and basis for the next step of early prevention intervention. Methods In this study, 102 patients after stroke depression(PSD) of the patients into the observation group, and randomly selected 100 cases without concurrent PSD of cerebral apoplexy patients into control group, patients in both groups of serum IL^(-1), IL^(-1)8, hs-CRP related inflammation factor were determined, and the relationship between the observation with SDS score. Results Through for two groups of patients serum IL^(-1), IL^(-1)8, hs-CRP determination and comparison found that inflammatory factors such as observation group of patients serum IL^(-1), IL^(-1)8, hs-CRP level obviously higher than the control group patients, the contrast of two groups of patients with statistically significant difference(P < 0.05), and the levels of inflammatory markers in patients with severe PSD related changes were higher than in patients with mild-to-moderate(P < 0.05). By Logistic regression analysis found that inflammatory factors, IL^(-1)8, hs-CRP IL^(-1) is a risk factor for depression occur after stroke(P < 0.05). Conclusion According to patients' serum IL^(-1), IL^(-1)8, hs-CRP level predict patients with depression after stroke, strengthen clinical observation, early detection, early treatment, can guide the clinical medication, leading to more effective treatment of poststroke depression, promote patients recover, improve the quality of life, has the high social and economic benefits.
