应用膝关节镜行外侧支持带松解术治疗髌骨复发性脱位32例

The Clinical Effect of Arthroscopic Lateral Knee Support for the Treatment of Recurrent Dislocation of the Patella

目的：观察膝关节镜下外侧支持带松解治疗髌骨复发性脱位的临床疗效.方法：2009年1月～2015年6月,本科对32例髌骨复发性脱位患者,借助膝关节镜,应用外侧支持带松解的方法进行治疗,对存在内侧支持带松弛现象者,将内侧支持带切开后重叠紧缩缝合,术后常规抗生素治疗共 5 天,12～14 天拆线,手术完成后加强膝关节功能锻炼.结果：本组32例患者住院时间平均为11.5±2.1天,手术切口均达甲级愈合.所有患者均获随访,为期1～5年,32例患者中达优者23例,达良者8例,判断为差者1例,优良率达96.9％.结论：借助膝关节镜,应用外侧支持带松解术治疗髌骨复发性脱位,操作简便,损伤小,疗效稳定可靠,对于膝关节功能恢复具有积极作用.

Objective： To observe the clinical effect of arthroscopic lateral knee support for the treatment of recurrent dislocation of the patella. Methods： From January 2009 to June 2015, undergraduate of 32 cases of recurrent patellar dislocation were treated, with knee arthroscopy, application of lateral retinaculum release solution treatment, on the existence of a medial support with relaxation phenomena, medial support with open after overlapping tight suture, postoperative routine antibiotic treatment for 5 days, stitches were removed during 12-14 days, after the completion of the operation to strengthen functional exercise of knee joint. Results： the average length of hospital stay was （11.5±2.1） days in 32 cases, and the incision was healed. All the patients were followed up for a period of 1-5 years; Among the 32 cases, 23 cases were excellent , 8 cases were good, 1 case was judged for poor. Excellent rehabilitation rate was 96.9％. Conclusion： with the help of knee arthroscopy, the use of lateral support for the treatment of recurrent dislocation of the patella, the operation is simple, the damage is small, the curative effect is stable and reliable, and has a positive effect on the recovery of knee function.
