地特胰岛素联合阿卡波糖治疗口服降糖药失效的老年2型糖尿病患者的疗效观察

Observation of clinical effect of insulin detemir combined with acarbose in treatment of elderly patients with type 2 diabetes mellitus and invalid oral hypoglycemic agents

目的探讨分析地特胰岛素联合阿卡波糖治疗口服降糖药失效的老年2型糖尿病疾病的临床疗效。方法随机选取新疆博州人民医院2013年4月-2015年4月期间接受治疗的102例老年2型糖尿病患者作为研究分析对象,所选患者均接受口服降糖药物治疗且失效,将102例患者按入院单双号分为对照组(51例)和研究组(51例),对照组患者接受地特胰岛素进行治疗,研究组则接受地特胰岛素联合阿卡波糖进行治疗,比较两组患者临床治疗状况。结果比较两组患者血糖指标状况,从Hb A1C、2h PBG、FBG等三个方面来看,研究组优于对照组,且组间数据有统计学意义(P<0.05)。比较两组患者24m A1b、达标时间、胰岛素用量等状况,研究组优于对照组,组间数据有统计学意义(P<0.05)。比较两组患者不良反应总发生率,研究组5.88%和对照组7.84%,组间差异无统计学意义(χ~2=0.3006,P=0.5834)。结论临床针对口服降糖药物治疗失效的老年2型糖尿病患者可再给予地特胰岛素联合阿卡波糖进行治疗,此治疗方式临床疗效显著,患者各指标状况得到更好的改善,其推广应用价值较大。

Objective To investigate and analyze the clinical effects of insulin detemir combined with acarbose in treatment of elderly patients with type 2 diabetes mellitus and oral hypoglycemie agents being invalid. Methods 102 elderly patients with type 2 diabetes who were accepted treatments in our hospital from April 2013 to April 2015 were randomly selected as the research objects, and all patients had been treated with oral hypoglycemic drugs and failed. 102 cases of patients were divided into control group （51 cases） and study group （51 eases） according to the admission order. The control group were received insulin detemir treatment, while study group were received insulin detemir combined with acarbose treatment. Then, the clinical effect in two groups were compared. Results The blood glucose status of patients in two groups were compared. HhA 1 C, 2hPBG, FBG in study group were better than those in control group. 24mAlb, recovering time, amounts of insulin in study group were better than those in control group （P〈0.05）. The incidence of adverse effects between two groups were compared, and there was no significant difference between the study group （5.88%） and the control group （7.84%） （ X＂=-0-3006, /1=-0.5834）. Conclusion Insulin detemir combined with acarbose treatment to elderly patients with type 2 diabetes and having oral hypoglycemic drug treatment failure are significant, and the indexes of patients are improved, thus its promotion and application value are great.
