UE不同评估方法在诊断前列腺良恶性病变中的应用

摘要 目的研究超声弹性成像技术(UE)在诊断前列腺良恶性病变中的可行性,比较不同评估计方法的诊断效能。方法回顾性分析2011年6月&2014年6月124例患者的临床资料。结果两种评分方法均能较好地判断前列腺疾病痛灶的性质。根据病理诊断结果显示,运用应变率比值法诊断前列腺肿瘤的灵敏度为93.14%、特异度为83.22%、阳性预测值为84.12%,阴性预测值为72.84%,准确性为82.37%;而弹性评分组的相应指标分别为89.20%,69.18%,76.67%,71.42%和75.13%。应变率比值法的特异度、阳性预测值和准确率组显著高于弹性评分法。结论UE技术能较好地应用于前列腺病变的鉴别和判断。与ES评分法相比,SR比值法有更高的鉴别效能。 Objective To explore the value of ultrasound elastography ( UE ) in the diagnosis of benign and malignant prostate diseases, and to compare the diagnostic value of strain ratio and 5-point scale criteria. Methods The clinical data of 124 patients from June 2011 to June 2014 were retrospectively analyzed. Results Both methods could distinguish benign and malignant lung lesions well, and the strain ratio method could also effectively distinguish various benign lung lesions. The sensitivity, specificity, positive predictive value, negative predictive value and accuracy of the strain ratio method were 93.14%, 83.22%, 84.12%, 72.84% and 82.37%, respectively, while the corresponding values of the elasticity score method were 89.20%, 69.18 %, 76.67%, 71.42% and 75.13%, respectively. There were significant differences between the two groups in the specificity and positive predictive value ( P〈0.05 ) . Conclusion Ultrasound elastography can be applied to diagnose and differentiate lung lesions. Compared with the elasticity score method, the strain ratio method plays an important role in discriminating and diagnosing different kinds of benign lesions.
