周围神经病变对2型糖尿病患者胃电图参数的影响 被引量:8

摘要 目的探讨周围神经病变对2型糖尿病(T2DM)患者胃电图参数的影响。方法将75例T2DM患者根据肌电图结果及周围神经病变临床症状分为无病变组31例、轻度病变组21例及中重度病变组23例。所有患者均行空腹及餐后胃电图检查,对3组的胃电图平均幅值(AP)、平均频率(DF)、反应面积(RA)、餐后/空腹功率比进行比较,并比较3组患者的体质指数、病程、FBG、糖化血红蛋白、血脂等指标。结果 3组患者体质指数、FBG、糖化血红蛋白、TG及LDL比较,差异无统计学意义(P>0.05),但无病变组患者的病程均短于其他两组(P<0.05),而轻度病变组与中重度病变组病程比较,差异无统计学意义(P>0.05)。3组患者的空腹AP、空腹RA、空腹及餐后DF比较,差异无统计学意义(P>0.05);无病变组、轻度病变组、中重度病变组餐后AP、餐后RA及餐后/空腹功率依次降低(P<0.05)。结论糖尿病周围神经病变的T2DM患者早期即可出现胃电图异常,而随着周围神经病变程度加重,患者胃电图异常更为明显。 Objective To investigate the effect of diabetic peripheral neuropathy on the parameters of electrogastrogram in patients with type 2 diabetes mellitus(T2DM). Methods A total of 75 patients with T2DM were divided into non-neuropatby group( u = 31 ) , mild neuropathy group (n = 21) and moderate to severe neuropatby group(n = 23 ) according to electromyogram and clinical symptoms of peripheral neuropathy. All patients underwent fasting and postprandial eleetrogastrogram. The average amplitude (AP) , average frequency (DF) and reaction area(RA) of electrogastrogram, and the ratio of postprandial to fasting power were compared among the three groups. The body mass index (BMI), duration of disease, FBG, glyeosylated hemoglobin and blood lipids were compared among the three groups. Results There were no significant differences in BMI, FBG,glycosylated hemoglobin,TG or LDL among the three groups(P 〉 0.05 ). The duration of disease in the non-neuropathy group was shorter than those in the other two groups(P 〈 0.05 ) , but no significant difference was found between the mild neuropathy group and the moderate to severe neuropathy group( P 〉 0.05 ). There were no significant differences in the fasting AP, fasting RA, fasting DF or postprandial DF among the three groups ( P 〉 0.05 ). The postprandial AP,postprandial RA, the ratio of postprandial to fasting power decreased in the order of non-neuropathy group,mild neuropathy group and moderate to severe neuropathy group ( P 〈 0.05 ). Conclusion Abnormal eleetrogastrogram occurs in the early stage in T2DM patients with diabetic peripheral neuropathy, and becomes more obvious with the increasing severity of peripheral neuropathy.
