复方苦参注射液腹腔灌注对恶性腹水患者临床疗效及对CEA、CA125、AFP水平的影响

Effect of Compound Kushen Injection Combined with Intraperitoneal Chemotherapy in the Treatment of Malignant Ascites and Its Effect on CEA,CA125 and AFP Levels

目的:探讨复方苦参注射液联合顺铂腹腔灌注治疗恶性腹水临床疗效及治疗前后肿瘤标志物CEA、CA125、AFP水平变化。方法:选取符合纳入标准的86例恶性腹水患者,随机分为对照组及观察组,每组43例,腹水引流完全后,对照组给予100 mg顺铂腹腔灌注,观察组给予100 mg顺铂+30 ml复方苦参注射液腹腔灌注,1次/周,连续给药3周;比较两组患者临床疗效、治疗前后胃肠道反应及肝肾功能变化,观察治疗前后CEA、CA125、AFP水平变化。结果:总有效率观察组高于对照组,但无统计学差异(P>0.05),治疗后CEA、CA125、AFP水平明显低于治疗前(P<0.05),两组CEA、CA125、AFP水平比较差异具有统计学意义(P<0.05),观察组不良反应发生率明显低于对照组(P<0.05)。结论:复方苦参注射液联合顺铂腹腔灌注治疗恶性腹水可有效改善患者临床症状,降低CEA、CA125、AFP水平,提高临床疗效,降低毒副反应。

Objective ： To investigate the clinical efficacy of compound Kushen injection combined with intraper- itoneal injection of cisplatin in the treatment of malignant ascites, and the changes of tumor marker CEA, CA125 and AFP before and after treatment. Methods ： A total of 86 patients with malignant ascites were selected and were randomly divided into a control group and an observation group, with 43 cases in each group. After ascites drainage, the patients in the control group were given 100 mg cisplatinperitoneal perfusion, while the patients in the observation group were given 100 mg cisplatin ＋ 30 ml compound Kushen injection perfusion, one time/week, with the continuous administration for three weeks. The clinical efficacy and the changes of gastrointestinal function, liver and kidney function before and after treatment were compared. The levels of CEA, CA125 and AFP were observed before and after treatment. Results ：The total effective rate in the obser- vation group was higher than that in the control group （P 〉 0.05 ）. After treatment the levels of CEA, CA125 and AFP in two groups were significantly lower than those before treatment （ P 〈 0.05 ）. The differences of the levels of CEA, CA125 and AFP in two groups were statistically significant （P 〈 0.05 ）. The incidences of adverse reactions in the observation group were lower than those in the control group （ P 〈 0.05 ）. Conclusion ： The results show that the combination of compound Kushen injection and intraperitoneal injection of cisplatin in the treatment of malignant the ascites can effectively improve the clinical symptoms, reduce the levels of CEA, CA125 and AFP, which can improve clinical efficacy and reduce side effects.
