综合护理干预对无创呼吸机治疗COPD合并呼吸衰竭患者的影响分析

Effect of Comprehensive Nursing Intervention on Patients with COPD Complicated with Respiratory Failure Treated with Noninvasive Ventilator

目的:分析综合护理干预对无创呼吸机治疗COPD(慢性阻塞性肺疾病,Chronic Obstructive Pulmonary Diseases)合并呼吸衰竭患者的影响。方法:选择2016年3月~2016年7月期间在某院治疗的30例慢性阻塞性肺疾病合并呼吸衰竭患者作为研究对象,将30例患者根据随机抽样法进行分组,每组各15例。两组患者均采用无创呼吸机治疗,对照组进行常规护理,而观察组在对照组的基础上采用综合护理干预。比较两组pH、PaO_2、SaO_2、PaCO_2改善情况、治疗依从性及不良反应。结果:观察组干预2h后PaO_2、SaO_2较干预前比较改善明显(P<0.05);观察组干预48h后pH、PaO_2、SaO_2、PaCO_2较干预前比较改善明显(P<0.05)。对照组干预48h后PaO_2、SaO_2、PaCO_2较干预前比较改善(P<0.05)。与对照组比较,观察组干预2h后PaO_2、SaO_2改善明显(P<0.05);观察组干预48h后pH、PaO_2、SaO_2、PaCO_2改善明显(P<0.05)。观察组治疗依从性明显高于对照组,并发症少于对照组(P<0.05)。结论:对慢性阻塞性肺疾病合并呼吸衰竭患者进行综合护理干预,有助于提高治疗效果,减少患者发生并发症和气管插管次数。

Objective：To explore the effect of comprehensive nursing intervention on patients with COPD （chronic obstructive pulmonary diseases）complicated with respiratory failure by noninvasive ventilator. Methods： 30 patients with chronic obstructive pulmonary disease （COPD） complicated with respiratory failure treated in a hospital from March 2016 to July 2016 were selected and divided into two groups according to the random sampling method, with 15 cases in each group. Both of two groups were treated with non-invasive ventilator ,while the control group received routine care, and the observation group received comprehensive nursing intervention on the basis of the control group.Then the improvement of pH, PaO2 ,SaO2, PaCO2,treatment compliance and adverse reactions were compared between the two groups.Results： After 2h of inter-vention,the PaO2 and SaO2 in the observation group were significantly improved compared with those before the intervention （P〈0.05）.After 48h of intervention,the PH, PaO2, SaO2 and PaCO2 improved significantly compared with those before the intervention （P 〈0.05）.After 48h of intervention, the PaO2, SaO2 and PaCO2 of control group were improved compared with those before the intervention （P〈0.05）.Compared with control group,the PaO2 and SaO2 of observation group improved significantly after 2h of intervention （P〈0.05） ;Compared with control group, the pH,PaO2 ,SaO2 ,PaCO2 of observation group improved significantly after 48 h of intervention （P〈0.05）.The treatment compliance of observation group was higher than that of control group, and the complication was less than that of control group （P〈0.05）.Conclusion：Comprehensive nursing intervention in patients with Chronic obstructive pulmonary disease complicated with respiratory failure can improve the therapeutic effect and reduce the complication and the number of tracheal intubation.
