摘要
目的:探讨应用包括个体化肠内营养的综合呼吸康复治疗对急性加重期慢阻肺患者的疗效及该治疗方案的应用价值。方法:本研究收集山西省汾阳医院2015年10月-2016年5月的80例慢性阻塞性肺疾病急性加重期(AECOPD)患者,随机分为试验组和对照组(每组40例)。试验组:肠内营养加呼吸康复治疗加常规治疗;对照组:常规治疗;入院当天及治疗14 d后测定肺功能,并对患者进行CAT评分,测定淋巴细胞绝对值。结果:两组AECOPD患者治疗后肺功能(FEV1、FVC)均得到改善,差异有统计学意义(P<0.01),其中试验组FEV1较对照组改善明显(P<0.01)。试验组患者治疗后CAT、淋巴细胞得到改善,差异有统计学意义(P<0.01),对照组患者治疗前后CAT、淋巴细胞比较,差异无统计学意义(P>0.05)。结论:COPD患者在急性加重期给予个体化肠内营养联合呼吸康复治疗,可有效改善肺功能,提高患者生活质量,值得借鉴。
Objective: To investigate the curative effect and the application value of individual enteral nutrition and comprehensive respiratory rehabilitation treatment for the patients with AECOPD.Method: 80 cases with AECOPD were selected in the Fenyang Hospital of Shanxi Province from October 2015 to May 2016, and the patients were randomly divided into control group and experimental group(40 cases in each group).The experimental group was treated with enteral nutrition, respiratory rehabilitation therapy and routine therapy; and the control group was treated with the conventional treatment.On the day of admission and after 14 days of treatment, the determination of pulmonary function, the CAT scores and the lymphocyte absolute value were made.Result: The pulmonary function(FEV1 and FVC) of the patients with AECOPD in the two groups were both improved after the treatment, the differences were statistically significant(P〈0.01), and the experimental group was improved more than the control group in FEVI(P〈0.01).The CAT scores and lymphocyte of the experimental group were improved after the treatment, and the differences were statistically significant(P〈0.01), but there were no significant differences in the control group after the treatment(P〉0.05).Conclusion: Giving individualized enteral nutrition with respiratory rehabilitation therapy to the patients with COPD at acute exacerbation period, can effectively improve the pulmonary function, improve the patients quality of life.lt is worth using for reference.
