多层螺旋CT在早期肺癌筛查中的临床应用价值 被引量:43

摘要 目的探讨多层螺旋CT扫描技术在早期肺癌筛查中的临床价值。方法选取早期肺癌患者160例,进行多层螺旋CT扫描诊断,观察CT片中患者肺部轮廓及病灶扫描图像,将CT诊断结果与病理检查结果比较,研究多层螺旋CT扫描技术在早期肺癌筛查中的准确率。结果多层螺旋CT扫描能清晰显示患者胸部各叶细节结构,肺部轮廓、病灶形状等。160例早期肺癌患者中,经过多层螺旋CT扫描诊断确诊151例,准确率为94.37%,与病理检查结果相比较,差异无统计学意义(P>0.05);72例周围型肺癌患者中,确诊71例,准确率98.61%;88例中央型肺癌中,确诊80例,准确率为90.90%。多层螺旋CT诊断周围型肺癌、中央型肺癌准确率差异有统计学意义(P<0.05)。结论多层螺旋CT扫描技术下,肺部组织轮廓、病灶显示清晰,在早期肺癌筛查中准确率较高,值得推广使用。但在中央型肺癌早期筛查时漏诊率相对较高,需要采用病理检查等其他方式辅助诊断,提高准确率。 Objective To study the clinical value of muhi-slice spiral CT scanning in the screening of early lung cancer. Methods 160 cases of early lung cancer were selected,multi-slice spiral CT scan was used in the diagnosis,and CT scan image of lung contour and lesions were observed, the CT diagnostic findings and pathological results were compared, accuracy of multi- slice spiral CT scanning technique in screening of early lung cancer were analyzed. Results Multi-slice spiral CT scan can clear- ly show the patients chest of each leaf details structure lung contour,lesion shape ; 160 cases of patients with early stage lung canc- er, after multi-slice spiral CT scan in the diagnosis of 151 cases, the accuracy rate was 94.37%, and pathological examination re- suits were compared, the difference was not statistically significance (P 〉 0.05 ) ;in 72 cases of peripheral lung cancer,71 cases were diagnosed, accuracy rate was 98.61% ;in 88 cases of central type lung cancer, diagnosed was 80 cases, the accuracy rate was 90.90%, and multi-slice spiral CT in the diagnosis of peripheral lung cancer and central type lung cancer accurate rate difference was statistically significant (P 〈 O. 05). Conclusion Multislice spiral CT scan technology, lung tissue contour, focus clear dis- play, in early lung cancer screening with a high accuracy, it is worthy to be popularized, but in early screening of the central type lung cancer missed diagnosis rate is relatively high, pathological examination and other auxiliary diagnosis method should be used to improve the accuracy.
