护理干预对妇科腹腔镜手术血栓前状态的影响研究

Efficacy of nursing intervention on prethrombotic state in gynecologic laparoscopic surgery

目的探讨护理干预对妇科腹腔镜手术血栓前状态的影响。方法选取2014年1月10日~2015年12月10日我院收治的600例实施腹腔镜手术的妇科患者,随机分为观察组与对照组,各300例。观察组患者采用预防性护理干预,对照组采用常规护理。比较两组患者的凝血酶原时间(PT)、活化部分凝血活酶时间(APTT)、纤维蛋白原(FIB)、疼痛情况、肠道排气时间及下床活动时间。结果观察组患者护理后的PT[(9.45±0.36)s]、APTT[(23.04±2.36)s]短于对照组[(11.02±0.52)、(25.74±2.51)s],FIB水平[(5.48±1.45)g/L]高于对照组[(3.58±1.03)g/L],疼痛率(1.67%)低于对照组(24.67%),下床活动时间[(2.36±0.45)d]、肠道排气时间[(1.03±0.06)d]短于对照组[(4.85±1.05)、(3.14±0.98)d],差异有统计学意义(P<0.05)。结论对妇科腹腔镜手术患者实施预防性护理干预治疗,能缩短患者PT及APTT,提高FIB水平,减轻患者疼痛,促进患者康复。

Objective To explore the efficacy of nursing intervention on prethrombotic state in gynecologic laparoscopic surgery.Methods 600 gynecological patients underwent laparoscopic surgery from January 10- 2014 to December 10th 2015 were selected and randomly divided into the observation group and the control group,with 300 cases in each group.Patients in the observation group were given preventive nursing intervention,while patients in the control group were given routine nursing.The FF,APTT,the level of FIB,the incidence of pain,the time of intestinal exhaust and activity time were compared between two groups.Results After nursing,the PT [（9.45±0.36）s] and AFI＇T[（23.04±2.36）s] in the observation group were shorter than those in the control group [（11.02±0.52）,（25.74±2.51）s],the level of FIB [（5.48±1.45）g/L] was higher than thai in the control group [（3.58±l.03）g/L],the incidence of pain （1.67%）was lower than that in the con- trol group（24.67%）,the time of intestinal exhaust [（2.36±0.45）d] and activity time [（1.03±0.06）d] were shorter than those in the control group [（4.85±l.05）,（3.14±0.98）d],and all the differences were statistically significant（P〈0.05）.Condusion The efficacy of nursing intervention on prethrombotic state in gynecologic laparoscopic surgery is definite.It can shorten the PT and FVF and can improve the level of FIB of patients,also can relieve pain and promote patients recover.
