医院社区联动自我管理教育模式对糖尿病患者的影响

Efficacy of hospital-community self-management education model on patients with diabetes mellitus

目的探索医院社区联动自我管理教育模式对糖尿病患者的影响。方法选取2015年9月~2016年9月我院收治的120例糖尿病患者,随机分为两组,各60例。对照组采用传统宣教模式,观察组采用医院社区联动自我管理教育模式。比较两组患者各代谢指标水平、生活质量、糖尿病知识掌握度。结果干预前两组患者随机血糖、总胆固醇、高密度脂蛋白、糖化血红蛋白水平比较,差异无统计学意义(P>0.05);干预后观察组各指标水平均低于对照组,差异有统计学意义(P<0.05)。观察组患者护理后生活质量量表及糖尿病知识掌握度各指标评分高于对照组,差异有统计学意义(P<0.05)。结论医院社区联动自我管理教育模式在糖尿病患者中效果显著,值得临床推广应用。

Objective To explore the efficacy of hospital-community self-management education model on patients with diabetes mellitus.nethods 120 patients with diabetes mellitus in our hospital from September 2015 and September 2016 were selected and randomly divided into the observation group and the control group.Patients in the control group were given traditional education mode,while patients in the observation group were given hospital-community self-management education model.The levels of metabolic indexes,quality of life and cognition degree of diabetes knowledge were compared between two groups.Results Before intervention,there were no significantly different among the levels of random blood glucose,total cholesterol,high-density lipoprotein cholesterol and glycosylated hemoglobin in two groups （P〉0.05）;after intervention,the levels of those in the observation group were lower than those in the control group,and the differences were statistically significant （P〈0.05）.The scores of all indexes in quality of life and cognition degree of diabetes knowledge in the observation group were higher than those in the control group,and the differences were statistically significant（P〈0.05）.Conclusion The hospital-community self-management education pattern in patients with diabetes is effective,and it is worthy of clinical application.
