两种化疗泵在结直肠癌患者持续化疗中应用的对比研究 被引量:3

摘要 目的比较微电脑全自动化疗泵和便携式化疗泵在结直肠癌患者化疗中的应用效果。方法选取自2015年3月至2016年3月在本科室行mFOLOFOX6方案化疗的结直肠癌患者80例,分为观察组和对照组,每组各40例,其中观察组运用微电脑全自动化疗泵输注5-FU,对照组使用便携式化疗泵输注5-FU。比较两组患者化疗不良反应发生情况、输注时间和护理满意度。结果对比便携式化疗泵,微电脑全自动化疗泵的操作更复杂,护士需要更长培训时间和更多培训次数来掌握其操作方法,输注过程中需排除的机器障碍次数更多(均P<0.05)。观察组患者1~2级化疗不良反应中口腔黏膜炎发生率低于对照组患者(12.5%vs.32.5%,χ~2=4.588,P=0.032)。观察组患者3~4级化疗不良反应中恶心呕吐发生率低于对照组患者(5.0%vs.22.5%,χ~2=5.165,P=0.023),两组其他化疗不良反应发生率比较差异均无统计学意义(均P>0.05)。观察组对比对照组的输注时间波动更小,更接近预定时间[(45.2±2.1)小时vs.(47.4±2.0)小时,P<0.001]。观察组患者护理满意度为优于对照组(95.0%vs.80.0%,χ~2=4.114,P=0.043)。结论结直肠癌患者化疗过程中应用微电脑全自动化疗泵可减轻化疗不良反应,减少输注时间误差,提高患者护理满意度。 Objective To compare the effect of microcomputer automatic chemotherapy pump and portable chemotherapy pump in the chemotherapy of colorectal cancer patients. Methods From March 2015 to March 2016,80 patients with colorectal cancer receiving mFOLFOX6 regimen chemotherapy were included. Patients were divided into two groups. There were 40 cases in each group. The observation group received 5-FU through microcomputer automatic chemotherapy pump, and the contrast group via portable chemotherapy pump. The incidence of chemotherapy toxicity, infusion time and patient satisfaction were compared between the two groups. Results Compared with the portable chemotherapy pump, the microcomputer automatic chemotherapy pump was more complicated. Longer time and more training was needed for nurses to master the operation method of microcomputer automatic chemotherapy pump. Meanwhile, there were more mechanical errors needed to be debugged during infusion (all P〈0.05). The incidence of the oral mucositis in Grade 1 -2 chemotherapy toxicity of the observation group was significantly lower than that of the contrast group (12.5% vs. 32.5%, χ2=4.588, P=0.032). The incidence of the nausea and vomiting in Grade 3 -4 chemotherapy toxicity of the observation group was significantly lower than that of the contrast group (5.0% vs. 22.5%,χ2=5.165, P=0.023). There was no significant difference in the incidence of other adverse reactions between the two groups (all P〉0.05). Compared with the contrast group, the fluctuation of infusion time in the observation group was smaller and more close to the scheduled time [(45.2±2.1)h vs. (47.4±2.0) h, P〈0.001]. The satisfaction degree of the observation group is better than that of the control group (95.0% vs. 80.0%, χ2=4.114, P=0.043). Conclusions The application of microcomputer automatic chemotherapypump in the treatment of colorectal cancer patients can reduce the toxicity of chemotherapy, reduce the time of infusion and improve the satisfaction degree of patients.
