摘要
目的:探讨声速匹配技术在评价肾功能不全肾实质纤维化中的价值。方法:使用超声的声速匹配成像技术检测95例受检者左肾实质,包括对照组30例,肾功能不全组32例,慢性肾衰组33例。分2步完成测量过程:首先调整取样框,包含部分肾实质,测得声速匹配值V1,然后缩小取样框,不包含肾实质,完全为肾周的结缔组织,测得声速匹配值V2,检查结束后,静态分析,分别测量取样框的面积S1和S2,利用公式计算三组间的肾实质的声速匹配值。结果:对照组、肾功能不全组和慢性肾衰竭组,三组间肾实质肾实质匹配值比较均P<0.05,有统计学差异;三组间肾周组织声速匹配值比较均P>0.05,无统计学差异;选区声速匹配指数对肾功能不全组检出率明显高于常规超声,P<0.001。结论:选区声速匹配指数技术可以有效检测肾功能不全肾脏纤维化程度。
Objective:To evaluate the value of zone speed index(ZSI)for dection renal fibrosis in patients with renal insufficency patients.Methods:95 cases accepted examination of left renal cortex by ZSI.According to the level of urea nitrogen and creatinine,cases were divided into three groups:group of control(30 cases),group of patients with renal insufficiency(32 cases),group of patients with uremia(33 cases).All the indecies obtained by two steps.V1 of ZSI was obtained through adjustion of the sampling frame which must include part of the renal parenchyma,while V2 of ZSI was obtained through adjustion of the sampling frame which include the connective tissue completely without any renal parenchyma.Then the area of the sampling framel(S1and S2)was measured.Results:Compared with group of control,ZSI of renal parenchyma decreased significantly in group of patients with renal insufficiency and uremia(P〈0.05),while there was no significant difference in ZSI of perirenal tissue among the three groups(P〉0.05).Compared with conventional ultrasound,the rate of detection for renal insufficiency increased significantly(P〈0.001).Conclusion:The ZSI technique could detect the renal fibrosis of patients with renal insufficiency effectively.
