茵栀黄口服液对新生儿黄疸特征标记物影响的研究分析 被引量:5

摘要 目的:探讨采用茵栀黄口服液治疗新生儿黄疸前后的代谢标记物变化情况。方法:通过随机对照试验,对75例新生儿黄疸的治疗情况分析,明确茵栀黄口服液的疗效,并分析治疗前后茵栀黄口服液的代谢标志物变化情况。结果:茵栀黄口服液经皮胆红素水平与蓝光治疗组无明显差异(P>0.05),通过代谢组学共鉴定鹅脱氧胆酸、L-犬尿氨酸、肌酐、L-酪氨酸、脱氧胆酸、甘胆酸、油酰胺、L-组氨酸共8个特征标志物,其中新生儿黄疸治疗前的鹅脱氧胆酸、脱氧胆酸、肌酐、甘胆酸含量较正常婴儿显著升高(P<0.01),L-酪氨酸、油酰胺、L-组氨酸较正常婴儿显著下降(P<0.05,P<0.01);通过茵栀黄口服液治疗后,代谢标记物均有不同程度地向正常婴儿含量靠近。结论:茵栀黄口服液可显著提高新生儿黄疸的治疗效果,其作用机制与改善鹅脱氧胆酸、L-犬尿氨酸、肌酐、L-酪氨酸、脱氧胆酸、甘胆酸、油酰胺、L-组氨酸共8个特征标志物有密切的关系。 Objective To investigate the effect of Yinzhihuang Oral Liquid(YOL) on the Characteristic Marker of Neonatal Jaundice. Method 75 cases of neonatal jaundice were randomly divided into two groups. One group was treated with YOL,anoth-er group was treated with Blue - ray. We compared the clinical efficacy of the two groups and investigated the effect of YOL on the Characteristic Marker of Neonatal Jaundice. Results The transcutaneous bilirubin of YOL had no difference between YOL and Blu - ray(P 〉0.05). By metabolomics, we identified eight characteristics markers, including chenodeoxycholic acid,L - kynure- nine, creatinine,L - tyrosine,deoxycholic acid, bile acid,oleic acid amide and L - histidine. Before therapy,the chenodeoxychol-ic acid, deoxycholic acid, creatinine and cholyglycine of neonatal jaundice was significantly higher than normal infants (P 〈0. 01 ).The L - tyrosine, oleamide and L - histidine of neonatal jaundice was significantly lower than normal infants (P 〈 0. 01 〈0. 05 ). After treatment, the characteristic Marker of Neonatal Jaundice was close to normal. Conclusion YOL had a good effect on neonatal jaundice. The mechanism may be associated with improving the CDCA,L - kynurenine,creatinine,L - tyrosine,deoxy-cholic acid, bile acid, oleic acid amide, L - histidine feature.
