穿刺引流在治疗哺乳期乳腺脓肿中的应用

Application of Puncture and Drainage in the Treatment for Lactation Breast Abscess

目的探讨穿刺引流在微创治疗哺乳期乳腺脓肿中的应用价值及可行性。方法选取2008-03—2014-10间在盐城市第六人民医院门诊治疗的82例哺乳期乳腺脓肿患者,其中穿刺引流(观察组)47例,切开引流(对照组)35例,术后通过观察治疗过程中的疼痛程度、抗生素使用天数、愈合时间、乳房外观满意度及复发率,采用Wilcoxon秩和检验比较两组治疗过程中的疼痛程度及乳房外观满意度,t检验比较两组抗生素使用天数、愈合时间及复发率。结果观察组治疗过程中疼痛程度轻于对照组(t=3.16,P<0.05),愈合时间较对照组明显缩短(:11.98天比21.97天,t=2.767,P<0.05),乳房外观满意度优于对照组(t=2.63,P<0.05),抗生素使用时间相近(:3.34天比3.26天,t=0.048,P=0.636>0.05),两组均无复发。结论穿刺引流治疗哺乳期乳腺脓肿操作简单,患者痛苦小,不延长抗生素使用天数,术后恢复快,瘢痕小,在临床应用中具有一定价值。

Objective To investigate the application value and feasibility of puncture and drainage in the minimally invasive treatment for lactation breast abscess. Methods A total of 82 outpatients diagnosed with lactation breast abscess were selected from March, 2008 to October, 2014, Among them, 47 patients were treated with puncture and drainage （observation group）, and others were treated with incision drainage （control group）. Statistical Methods were used to analyze if there were differences between these two groups in terms of the intraoperative pain （Wilcoxon test）, the time of antibiotic usage （t test） and wound healing （t test）, the satisfaction of patients＇ breast appearance （ Wilcoxon test）, the rate of recurrence （ t test）. Results The intraoperative pain in ob- servation group was lower than control group （t = 3. 16, P 〈 0. 05 ）, the time of wound healing in observation group was obviously shorter than control group （ 11.98 d vs 21.97d, t = 2. 767, P 〈 0. 05 ） , patients in the observation group were more satisfied with their breast appearance compared with control group （ t = 2. 63, P 〈 0. 05 ）, while the time of antibiotic usage between two groups were no statistical significance （ 3.34 d vs 3. 26d, t = 0. 048, P = 0. 636 〉 0. 05）. All of two groups had no recurrence. Conclusion Puncture and drainage has the advantages of simple operation, less pain, rapid recovery, small sear and no extra antibiotic usage for lactation breast abscess patients.
