摘要 目的:联合检测甲状腺乳头状癌组织中表皮生长因子受体(EGFR)和增殖细胞核抗原(PCNA)的表达及其相关性,并探讨其临床意义。方法:选取本院2010年9月至2015年12月经病理学确诊为甲状腺乳头状癌样本和癌旁组织样本各80例,病例临床资料完整。采用免疫组织化学方法检测甲状腺乳头状癌组织及癌旁组织中EGFR和PCNA的表达,并分析其临床病理特征与甲状腺乳头状癌中EGFR和PCNA表达情况的相关性。结果:EGFR和PCNA在甲状腺乳头状癌组织中的阳性表达率分别为78.75%和88.75%,在癌旁组织中的阳性表达率分别为35.00%和38.75%,差异均有统计学意义(P<0.05)。EGFR和PCNA在有淋巴结转移与无淋巴结转移的甲状腺乳头状癌组织表达情况比较差异有统计学意义(P<0.05);EGFR和PCNA在不同年龄、性别、临床分期和有无家族史的甲状腺乳头状癌组织中的阳性表达率比较差异均无统计学意义(P>0.05)。EGFR和PCNA在甲状腺乳头状癌组织中的表达呈正相关(r=1,P<0.01)。结论:EGFR和PCNA均有可能作为鉴别甲状腺组织良恶性病变的有意义指标。甲状腺乳头状癌组织中EGFR和PCNA的表达情况与其伴有淋巴结转移存在相关性,且EGFR和PCNA在甲状腺乳头状癌组织中的表达呈正相关。 Objective: To detect the expression of EGFR and PCNA in papillary thyroid carcinoma tissues and to explore their clinical significances. Methods: From Sep 2010 to Dec 2015, 80 cases of papillary thyroid carcinoma tissues diagnosed by pathology and 80 cases of corresponding para-cancerous tissues were enrolled. The clinical data of patients with thyroid carcinoma were collected. Immunohistochemical method was used to detect the expression of EGFR and PCNA in papillary thyroid carcinoma tissues and para-cancerous tissues. The correlations between clinical pathological characteristics and the expression of EGFR and PCNA were analyzed. Results: The positive expression rates of EGFR and PCNA in papillary thyroid carcinoma tissues were 78.75% and 88.75% respectively, 35.00% and 38.75% in para-cancerous tissues, there were statistically significant differences (P 〈 0.05). The positive expression rates of EGFR and PCNA in papillary thyroid carcinoma tissues with lymph node metastasis were significantly higher than those in papillary thyroid carcinoma tissues without lymph node metastasis (P 〈 0.05). The difference of the expression of EGFR and PCNA had no relatiorship with the patients' age, gender, clinical stage and family history of disease (P〉 0.05). There was positive correlation between EGFR and PCNA expression in papillary thyroid carcinoma tissues (r = 1, P〈0.01 ). Conclusion: EGFR and PCNA are likely as meaningful indicator of differential diagnosis of benign and malignant thyroid lesions. The expression of EGFR and PCNA in papillary thyroid carcinoma tissues are correlated with lymph node metastasis. The expression of EGFR and PCNA in papillary thyroid carcinoma tissues is positively correlated.
