窄谱中波紫外线对寻常型银屑病患者血清肿瘤坏死因子-α、白介素-8水平的影响

Effects of narrow-spectrum medium-wave ultraviolet on the levels of serum tumor necrosis factor-α and interleukin-8 in patients with psoriasis vulgaris

目的探讨窄谱中波紫外线对寻常型银屑病患者血清肿瘤坏死因子-α、白介素-8水平的影响。方法将2014年12月~2016年12月于我院治疗的寻常型银屑病患者66例纳入本研究,随机分为观察组(n=33)和对照组(n=33);对照组应用迪银片治疗,观察组应用窄谱中波紫外线治疗,比较两组患者治疗前后TNF-α、IL-8水平的变化情况、两组患者治疗前后PASI评分变化,并对两组的临床疗效进行评价。结果治疗前两组患者的TNF-α、IL-8水平比较,差异无统计学意义(P>0.05);治疗后,两组患者的TNF-α、IL-8水平均显著降低,且观察组较对照组降低更显著(P<0.05)。两组患者治疗前PASI评分比较,差异无统计学意义(P>0.05);治疗后,两组患者PASI评分较治疗前显著降低,组间比较差异有显著性(P<0.05);且观察组患者PASI评分治疗后显著低于对照组,差异具有显著性(P<0.05)。对照组治疗后的总有效率为78.8%,观察组治疗后的总有效率为97.0%,两组比较差异显著(χ~2=5.386,P<0.05)。结论窄谱中波紫外线通过降低寻常型银屑病患者TNF-a、IL-8水平而发挥抗炎作用,从而提高临床疗效。

Objective To investigate the effect of narrow-spectrum medium-wave ultraviolet on the levels of serum tu- mor necrosis factor-α and interIeukin-8 in patients with psoriasis vulgaris. Methods 66 patients with psoriasis vulgaris treated in our hospital from December 2014 to December 2016 were enrolled and randomly divided into observation group （n=33） and control group （n=33）. The control group was treated with Diyin tablets. The observation group was given narrow-spectrum medium-wave ultraviolet. The levels of TNF-α and IL-8 and the changes of PASI score be- tween the two groups were compared before and after treatment. The clinical efficacy of the two groups was evaluated. Results The levels of TNF-α and IL-8 were compared between the two groups before treatment, the differences were no statistically significant（P〉0.05）. The levels of TNF-α and IL-8 in the two groups were significantly lower than those before treatment, and the decrease of the observation group were more significant than those in the control group （P〈0.05）. The difference was no statistically significant in PASI score before treatment between both groups（P〉0.05）. PASI score of the two groups was significantly lower than that before treatment （P〈0.05）. After treatment PASI score in the observation group, which was significantly lower than that in the control group（P〈0.05）.The total effective rate was 78.8% in the control group, and that of 97.0% in the observation group after treatment, and the difference was signifi- cant（x2=5.386,P〈0.05）. Conclusion Narrow-spectrum medium＇wave ultraviolet can improve the clinical efficacy by reducing the levels of TNF-α and IL-8 in patients with psoriasis vulgaris.
