基于闭环管理的压疮系统构建

Construction of pressure sore system based on closed loop management

目的优化压疮管理流程,做到环节管理及质量持续改进。方法在医院信息系统(HIS)建立压疮管理模块,与6级电子病历有效结合,通过压疮监控模块筛查高危患者并进行预警提示,对不同来源压疮数据信息上报,并记录治疗护理措施、压疮愈合等情况。结果实施压疮闭环管理后,2013~2015年压疮发生率分别为0.06%、0.06%、0.08%,低于国内12所医院的调研结果。结论压疮闭环管理体系的构建能够科学分析压疮发生现状,对治疗护理过程中存在的问题进行整改,进而提高压疮护理和管理质量。

Objective To optimize the workflow of pressure ulcer management, and to achieve process control and continuous quality improvement. Methods We inserted a module of pressure ulcer management into hospital information system（HIS）, and made it ef- fectively link to electronic medical record （EMR）. We screened patients at high risk of pressure ulcer through the module, which then set off an alarm. The module was also to able report pressure ulcer data across the hospital, and document nursing measures for and outcomes of pressure ulcer. Results After closed loop management, the incidence rates of pressure ulcer from 2013 through 2015 were 0.06% ,0.06% and 0. 08% respectively, all being lower than the result from an investigation into 12 domiciliary hospi tals. Conclusion Closed loop management enables nurses to scientifically analyze the incidence rates of pressure ulcer, and take ac- tive measures to solve the problems lying in nursing process, thus enhance the quality of pressure ulcer care.
